PHASE I ENVIRONMENTAL SITE ASSESSMENT 1454 MERIVALE, OTTAWA, ON Executive Summary Golder Associates Ltd. (Golder) was retained by First Capital Asset Management LP (FCAM) to upgrade a Phase I Environmental Site Assessment (Phase I ESA) completed by Pinchin Environmental (Pinchin) in 2013 (in accordance with the 2001 Canadian Standards Association (CSA) document entitled Phase I Environmental Site Assessment, Z768-01) for the property located at 1454 Merivale Road in Ottawa, Ontario (the Site) to meet the requirements for Phase I ESA as stipulated in the Ontario Regulation 153/04 (O.Reg. 153/04) as amended. The Site is developed with a two-storey commercial office building built in 1984 with a total area of 360 m2 (3,865 ft2) occupied by Bank of Montreal (BMO). The total area of the Site is 0.18 hectares (0.45 acres). This Phase I ESA was completed in accordance with the O.Reg. 153/04 (as amended) and is an upgrade of the Phase I ESA completed in accordance with the CSA document entitled Phase I ESA, Z768 01 (R2012) conducted for the Site in May 2013 for BMO by Pinchin. It is understood that this Phase I ESA is being carried out for Site Plan Approval purposes with the City of Ottawa (the City) and that a Record of Site Condition (RSC) pursuant to O. Reg. 153/04 – Records of Site Condition – Part XV.1, made under the Environmental Protection Act, will not be filed for the Site. The 2013 Phase I ESA was upgraded to meet the Phase I ESA requirements as stipulated in O.Reg. 153/04 (as amended) in order to comply with the City’s current Official Plan requirements that all Phase I ESA reports that are submitted for Site Plan Approval have to be completed in accordance with the O.Reg.153/04 as amended. The Executive Summary highlights key points from the report only; for complete information and findings, as well as the limitations, the reader should examine the complete report. The Phase I ESA was completed in accordance with the requirements of Schedule D of O.Reg.153/04 as amended and included a review of available current and historical information regarding the Site and surrounding properties, a Site reconnaissance, interviews, evaluation of readily available information, and reporting, subject to the limitations outlined in Section 8.0 of this report. The Site is not considered an enhanced investigation property as defined by O. Reg. 153/04. Based on the information provided by the Site owner and the City of Ottawa geo-map, the legal description of the Site is PLAN 30 S PT LOT 16, City of Ottawa. The Site municipal address is 1454 Merivale Road, Ottawa, Ontario. At the time of the Site visit on February 6, 2014, the Site was developed with a two-storey commercial office building built in 1984 and was occupied by Bank of Montreal. Office related activities were being carried out at Site. Historically, the Site was vacant until 1984 when the Site was develop with the current commercial office building occupied by BMO. Between late 1960s and 1984 the Site was used as a parking lot and prior to 1960s it was vacant land overgrown with vegetation. At the time of the Phase I ESA, the neighbouring properties within the Phase One Study Area were used for commercial and residential purposes. Based on the information collected as part of this Phase One ESA, no areas of potential environmental concern (APECs) were identified on the Site and no potentially contaminating activities (PCAs) were identified on the Site or on the Phase I ESA Study Area that might have resulted in APECs on the Site. The following is recommended for the Site: If the monitoring wells completed as part of the 2013 Golder Phase II ESA will be no longer used, they should be decommissioned as per O. Reg. 903. However, it is considered that it will be beneficial for FCAP to keep the monitoring wells for future groundwater sampling to assess if the groundwater quality at the Site has changed since the 2013 Golder Phase II ESA. This need for basic school supplies is linked directly with lower self- esteem in students and contributes to diminished school performance for children and youth in our communities. The Tools 4 School program collects the much needed supplies for school-aged children in the Ottawa Community through donations, community wide supply drives, strong partnerships and corporate challenges. Every year, the Tools 4 School program distributes school supplies to children and youth in need at the start of the school year (September 2015).

    REASSESSMENT OF THE RESPONSE TO TSB RECOMMENDATION R15-05 Grade separation of Woodroffe Avenue, the Transitway, and Fallowfield Road Background On 18 September 2013, at about 0832 Eastern Daylight Time, westward VIA Rail Canada Inc. (VIA) passenger train No. 51 departed from the VIA Ottawa Station on time and proceeded enroute to Toronto. At 0847:27, OC Transpo double-decker bus No. 8017 departed from the Fallowfield Station on the OC Transpo bus Transitway. At 0848:06, while proceeding at about 43 mph, the train entered the OC Transpo Transitway crossing, located at Mile 3.30 of VIA’s Smiths Falls Subdivision. At the time, the crossing lights, bells and gates were activated. The northbound bus was travelling at about 5 mph with the brakes applied when it struck the train. As a result of the collision, the front of the bus was torn off. The train, comprising 1 locomotive and 4 passenger cars, derailed but remained upright. Among the bus occupants, there were 6 fatalities and 9 serious injuries, and about 25 minor injuries were reported. No VIA crew members or VIA passengers were injured. The Board concluded its investigation and released report R13T0192 on 2 December 2015. TSB Recommendation R15-05 (December 2015) Originally, grade separations had been planned for Woodroffe Avenue, the Transitway, and Fallowfield Road. The plans were based on environmental assessments (EAs) conducted in the late 1990s. At that time, members of the public were opposed to any roadway overpass structure, and the National Capital Commission (NCC) supported the public position. Consequently, overpass options were not considered as part of the EAs.
