Safety and Tolerability of Arimoclomol in Patients with Sporadic Inclusion Body Myositis: a Randomised, Double-Blind, Placebo-Controlled, Proof-Of-Concept Trial
Safety and tolerability of arimoclomol in patients with sporadic inclusion body myositis: a randomised, double-blind, placebo-controlled, proof-of-concept trial Sporadic inclusion body myositis (IBM) is the commonest idiopathic inflammatory myopathy (IIM) occurring in patients over the age of 50 years.1-4 The prevalence of IBM differs between different populations and ethnic groups, with estimates between 4.9-70.6 per million population.5-10 IBM is distinguished from the other IIM by early asymmetric finger flexor and knee extensor weakness (leading to loss of hand function and propensity to fall) and resistance to immunosuppressive therapy. Any skeletal muscle can be affected, including oesophageal and pharyngeal muscles. Late-stage disease can be characterized by very significant morbidity including disability and loss of quality of life. Death in IBM is related to malnutrition, cachexia, aspiration, respiratory infection and respiratory failure, as a consequence of dysphagia, severe global weakness and weakness of the respiratory muscles.1-4 In a Dutch cohort, end-of-life care interventions were used by 13% of patients with IBM, highlighting the morbidity experienced by these patients and the need of supportive and palliative care in this disease.2 The aetiopathogenesis of IBM remains uncertain. The varied pathological findings observed have driven a number of theories including viral infection, accumulation of toxic proteins, autoimmune attack, myonuclear degeneration, endoplasmic reticulum stress and impairment of autophagy and proteasomal proteolysis.11-15 Muscle biopsies from patients with IBM typically show several different pathological features broadly described as inflammatory or degenerative. Inflammatory features include endomysial infiltration by mononuclear cells (predominantly CD8+ T- cells), which surround and invade otherwise normal appearing muscle fibres, and overexpression of major histocompatibility complex (MHC) class I, which is not constitutively expressed by skeletal muscle. -

Edaravone is a Memory potent free radical scavenger having antioxidant effect. In the current study, edaravone's ability to prevent SD Maze induced cognitive impairment was tested in rats. Animals were sleep deprived 8 h/day for 4 weeks. Oxidative stress Concurrently, edaravone was administrated intraperitoneally for four weeks. Animals performance during cognitive testing was evaluated to display if edaravone has a role in the prevention of sleep deprivation induced memory impairment. Additionally, the role of antioxidant biomarkers glutathione peroxidase (GPx), catalase, glutathione (GSH), oxidized glutathione (GSSG), GSH/GSSG in this effect was investigated. The results showed that SD impaired both short- and long- term memories, and chronic edaravone administration prevented such effect. Additionally, edaravone prevented decreases in hippocampal GPx, catalase, GSH/GSSG ratio and nor- malized increases in GSSG levels, which were impaired by SD model. In conclusion, current result showed a protective effect of edaravone administration against SD induction that could be related to edaravone's ability to normalizing mechanisms related to oxidative balance. -

Our previous studies have demonstrated that TCDD imprints sexual immaturity by suppressing the expression of fetal pituitary gonadotropins, the regulators of gonadal steroidogenesis. In the present study, we discovered that all TCDD-produced damage to fetal production of pituitary gonadotropins as well as testicular steroidogenesis can be repaired by co-treating pregnant rats with a-lipoic acid (LA), an obligate co-factor for intermediary metabolism including energy production. While LA also acts as an anti-oxidant, other anti-oxidants; i.e., ascorbic acid, butylated hydroxyanisole and edaravone, failed to exhibit any beneficial effects. Neither wasting syndrome nor CYP1A1 induction in the fetal brain caused through the activation of aryl hydrocarbon receptor (AhR) could be attenuated by LA. These lines of evidence suggest that oxidative stress makes only a minor contribution to the TCDD-induced disorder of fetal steroidogenesis, and LA has a restorative effect by targeting on mechanism(s) other than AhR activation. -

Turner 4 Abstract | Individuals who are diagnosed with amyotrophic lateral sclerosis (ALS) today face the same historically intransigent problem that has existed since the initial description of the disease in the 1860s — a lack of effective therapies. In part, the development of new treatments has been hampered by an imperfect understanding of the biological processes that trigger ALS and promote disease progression. Advances in our understanding of these biological processes, including the causative genetic mutations, and of the influence of environmental factors have deepened our appreciation of disease pathophysiology. The consequent identification of pathogenic targets means that the introduction of effective therapies is becoming a realistic prospect. Progress in precision medicine, including genetically targeted therapies, will undoubtedly change the natural history of ALS. The evolution of clinical trial designs combined with improved methods for patient stratification will facilitate the translation of novel therapies into the clinic. In addition, the refinement of emerging biomarkers of therapeutic benefits is critical to the streamlining of care for individuals. In this Review, we synthesize these developments in ALS and discuss the further developments and refinements needed to accelerate the introduction of effective therapeutic approaches. Neurodegeneration and dementia pose a major public owing to a lack of proven biomarkers and clinical trial health challenge worldwide owing to their devastating outcome measures10. -

The molecular and cellular aetiology of the spectrum is a highly complex encompassing dysfunction in many processes, includ- ing mitochondrial dysfunction and oxidative stress. There is a paucity of treatment options aside from therapies with subtle effects on the post diagnostic lifespan and symptom management. This presents great interest and necessity for the discovery and development of new compounds and therapies with beneficial effects on the disease. Polyphenols are secondary metabolites found in plant-based foods and are well known for their antioxidant activity. Recent research suggests that they also have a diverse array of neuroprotective functions that could lead to better treatments for neurodegenerative diseases. We present an overview of the effects of various polyphenols in cell line and animal models of ALS/FTD. Furthermore, possible mechanisms behind actions of the most researched compounds (resveratrol, curcumin and green tea catechins) are discussed.
