Methods: A multicenter prospective phase II trial was conducted to investigate the role of the combination of gemcitabine plus romidepsin (GEMRO regimen) in relapsed/refractory PTCL, looking for a potential synergistic effect of the two drugs. GEMRO regimen contemplates an induction with romidepsin plus gemcitabine for six 28-day cycles followed by maintenance with romidepsin for patients in at least partial remission. The primary endpoint was the overall response rate (ORR); secondary endpoints were survival, duration of response, and safety of the regimen. Results: The ORR was 30 % (6/20) with 15 % (3) complete response (CR) rate. Two-year overall survival was 50 % and progression-free survival 11.2 %. Grade ≥3 adverse events were represented by thrombocytopenia (60 %), neutropenia (50 %), and anemia (20 %). Two patients are still in CR with median response duration of 18 months. The majority of non-hematological toxicities were mild and transient.

Chlorambucil is probably mutagenic and teratogenic in humans. 9 Chlorambucil produces human infertility (see WARNINGS and PRECAUTIONS). 10 DESCRIPTION 11 LEUKERAN (chlorambucil) was first synthesized by Everett et al. It is a bifunctional 12 alkylating agent of the nitrogen mustard type that has been found active against selected human 13 neoplastic diseases. Chlorambucil is known chemically as 4-[bis(2- 14 chlorethyl)amino]benzenebutanoic acid and has the following structural formula: 15 16 17 18 Chlorambucil hydrolyzes in water and has a pKa of 5.8. 19 LEUKERAN (chlorambucil) is available in tablet form for oral administration. After single 25 oral doses of 0.6 to 1.2 mg/kg, peak plasma chlorambucil levels (Cmax) are reached within 1 hour 26 and the terminal elimination half-life (t½) of the parent drug is estimated at 1.5 hours. 27 Chlorambucil undergoes rapid metabolism to phenylacetic acid mustard, the major metabolite, 28 and the combined chlorambucil and phenylacetic acid mustard urinary excretion is extremely 29 low — less than 1% in 24 hours. The accurate discrimination between tumor and normal tissue is crucial for determining how much to resect and therefore for the clinical outcome of patients with brain tumors. In recent years, guidance with 5-aminolev- ulinic acid (5-ALA)–induced intraoperative fluorescence has proven to be a useful surgical adjunct for gross-total resection of high-grade gliomas. The clinical utility of 5-ALA in resection of brain tumors other than glioblastomas has not yet been established. The authors assessed the frequency of positive 5-ALA fluorescence in a cohort of pa- tients with primary brain tumors and metastases. Methods. The authors conducted a single-center retrospective analysis of 531 patients with intracranial tumors treated by 5-ALA–guided resection or biopsy. They analyzed patient characteristics, preoperative and postoperative liver function test results, intraoperative tumor fluorescence, and histological data. They also screened discharge summaries for clinical adverse effects resulting from the administration of 5-ALA. Intraoperative qualitative 5-ALA fluorescence (none, mild, moderate, and strong) was documented by the surgeon and dichotomized into negative and positive fluorescence.

These warnings include: - Vincristine should only be administered by the intravenous (IV) route. Administration of vincristine by any other route can be fatal. - Syringes containing this product should be labeled “Warning - for IV use only.” - Extemporaneously prepared syringes containing this product must be packaged in an overwrap which is labeled “Do not remove covering until moment of injection. For IV use only - fatal if given by other routes.” - Contraindication of vincristine in patients with demyelinating Charcot-Marie-Tooth syndrome. - Potential risk of acute shortness of breath when vincristine is coadministered with mitomycin-C and GI toxicities including necrosis with administration of vincristine.
