Prenatal Care and Delivery – Berkeley SHIP Referrals Fall 2017/Spring 2018
A Tang Social Worker (642-6074) can help you with general financing and insurance questions. For more detailed information, please visit the Student Health Insurance Office (642-5700) or the following web site: http://uhs.berkeley.edu/students/insurance/. Authorization for care is needed for insurance coverage through the Berkeley Student Health Insurance Plan and is available from Social Services or an Advice Nurse. When you schedule your appointment with one of the providers listed below, identify yourself as a UC Berkeley student and verify that the physician is currently an Anthem Blue Cross provider.
Berkeley SHIP Prenatal Coverage Delivery must occur while you are a student enrolled in Berkeley SHIP. The plan year runs August 1st through July 31st. Please see the benefits summary available from the Berkeley Student Health Insurance Office or the website for details of your coverage. There is a $15 co-pay (deductible waived) for your first prenatal visit; coverage is 100% for subsequent visits for in-network providers. Delivery, all labs, testing, etc. are covered at 90% in-network, $300 deductible applies.
Newborn Coverage: A child born to a covered SHIP member is covered for the first 31 Days of Life or until the student’s coverage ends whichever comes first. To enroll a newborn, please contact the Student Health Insurance Office at (510) 642-5700 within 48 hours after the birth of the baby. Dependent coverage after the first 31 Days of Life is offered for your newborn through Berkeley SHIP. Please contact the Student Health Insurance Office for benefit enrollment details or visit uhs.berkeley.edu/dependents. If you are ready to enroll your newborn, contact Wells Fargo at 1-800-853-5899.
