This guide is published by the Moorpark Active Adult Center, City of Moorpark. The guide is designed to provide interested persons with information about Adult Day Care facilities in Moorpark and the surrounding communities. This guide is a basic list of Adult Day Care facilities and does not imply endorsement. The listings are not all-inclusive and are reported as accurately as possible for your use and establishment of effective references. Failure to list an facility implies no criticism. What is adult day care?

Adult care provides activities in a supervised environment for older adults with , , depression or loneliness. Centers provide a variety of activities in a secure environment, including art, music and . Snacks or meals are usually served. Programs are designed to benefit both participant and caregiver, providing stimulation for participant and much-needed break for the caregiver.

Adult day care is tax deductible.

There are three types of adult day care centers.

Social Day Care Centers are primarily recreational. These centers provide supervised setting for group activities. Social Day Care Centers may accept persons with dementia, but may not accept someone who wanders, is incontinent or aggressive. Social Day Care Centers which specialize in programs for people with Alzheimer’s or a related dementia are usually equipped to handle wanderers, incontinence and aggressive behavior. Families pay privately for this type of day care program; Social Day Care Centers are not covered by Medicare or Medi-Cal.

Adult Day Health Care Centers (ADHC) specialize in providing medical, rehabilitation and social services for adults age 18 and up, who are physically or mentally impaired. ADHC services are provided by nurses; physical, speech and occupational therapists; social workers and other medical personnel. The Medi- Cal program will cover these services for low income persons. Some persons with dementia may be eligible for this program if there are physical impairments.

Adult Day Treatment Centers specialize in treatment of persons with psychological problems and may accept persons with dementia. Centers provide social and recreational programs, as well as medication management for problem behaviors. Various types of counseling. Some Medicare/Medi-Cal reimbursement (time-limited basis)

Camarillo

Description: A licensed day support program for adults who could benefit from day supervision or who lack opportunities for social interaction. Ages 18 and up. Provides opportunities for socialization, recreation and intellectual stimulation in a safe, supervised, friendly and supportive environment. Open Monday through Friday, 9 a.m. – 3 p.m. Participants may attend full days, half days, and as many or as few days of the week as desired. Wheelchairs and walkers are welcome. Pre-registration is required. participants may refer themselves or be referred through family, friends, social or health agencies, physicians, or clergy. Ojai

Help of Ojai’s Oak Tree House is a social model Adult Day Program that offers to the dependent elderly , fun and activities with peers.

Oak Tree House provides a secure environment in which cognitively or physically impaired seniors who require close supervision can nonetheless maintain critical social interactions and a degree of activity in their own community. The program also allows respite for that need to rest, regroup, or attend to other obligations.

Oak Tree House is open Monday, Wednesday, Thursday and Friday from 9:00 AM to 3:00 PM. Van transportation is provided, and snacks and a noon meal are served to all participants. Trained staff and volunteers coordinate an array of activities, including baking, chair , puzzles, games, parties, music, sing-along, pot gardening, and pet care. Discussion groups, life review and peer counseling are also available, as is the opportunity for relaxing and deck lounging.

Applicants must be able to care for themselves with minimal assistance. Contributions, upon which the program heavily depends, are based on individual circumstances. No one will be denied service due to race, color, creed, ethnic origin or ability to pay.

Oxnard

Description: Community program that offers social and health services to adults with physical cognitive psychosocial impairments. Eligible seniors are those 65 years of age and older with various medical problems, as well as younger adults with . The center provides a wide scope of services that include: a light breakfast, complete hot lunch and snack, nutritional counseling; transportation to and from the center; nursing supervision; physical and occupational maintenance ; social services; recreational and social activities, field trips and ESL classes. Staff includes social workers, registered nurses, physical, occupational and speech therapists, activity coordinator, consulting registered dietician, pharmacist and medical director. Serves Port Hueneme ,Oxnard, Camarillo and Ventura. Inquiries from other areas accepted.

Fees: No charge for persons on Medi-Cal; private pay for others.

Description: Provides care to seniors who are ambulatory and are able to maintain the skills required in daily life who are age 60 or older. Provides daytime supervision, companionship and motivation. Open Monday, Tuesday and Wednesday 10 a.m. – 3 p.m. Skilled, experienced staff. Lunch provided daily. Program activities include arts and crafts, music, gardening, physical activities. Collaborates with St. John’s Hospital; nurse visits program monthly and provides and blood sugar checks.

Fees: Bi-lingual Program. Sliding scale fee.

Description: The center is available for persons over the age of 18 with physical and/or cognitive impairment that interferes with daily functioning. Center provides medical care by registered nurses including medication management and general health care assessment; socialization; Speech, physical and occupational therapy; dietary counseling, social work services; individual and group counseling; psychology services; ; information about available community resources for seniors/persons with disabilities; ESL language services; memory enhancement classes, physician and podiatry services; massage treatments; hot nutritious meals and snacks. Regular community outings are provided, as well as special events and celebrations within the facility. Transportation is provided (within service area boundaries) by the center Monday - Friday. Service areas include Oxnard, Ventura, Camarillo, Point Magu, Port Hueneme, and Santa Paula. Licensed by the State Department of Aging and State Department of Health Services. Multi cultural programming and multi lingual staff.

Fees: All fees covered by Medi Cal for eligible persons, and qualified veterans; scan (Medicare HMO); private pay also accepted. Fees are determined on a sliding scale, based on ability to pay for private paying individuals.

Description: A community-based program providing a variety of medical, therapeutic, rehabilitative and recreational programs and services for adults 18 years of age and older. Program is designed to help adults age 18 and older who require assistance in the maintenance of their health care to either improve or maintain their activities of daily living (ADL’s). Assists adults with disabilities and forms of dementia limiting their ability to function independently. Services include, but are not limited to: assessments; nursing services/health screening; nutritional counseling; physical, occupational and speech ; social services; academic and educational classes; individual and group counseling; physical, social and recreational programs; nutritional hot meals and snacks; monthly field trips; two-way transportation (within service area boundaries); wheelchair accessibility. Service area includes Camarillo, El Rio, Fillmore, Oxnard, Point Mugu, Port Hueneme, Santa Paula, Saticoy, Somis and Ventura. Center is open Monday through Friday from 8 a.m. – 5 p.m., excluding major holidays. Licensed by the State of California Department of Health Services.

Fees: Services reimbursable through payments from Medi-Cal or private payments. Fees are determined on a sliding scale, based on ability to pay.

Simi Valley

Description: A community-based day program providing a variety of health, therapeutic, and social services designed to achieve and maintain optimal health. Provides scheduled activities and programs for participants. Program open from 8 a.m. to 5 p.m. Monday through Friday and serves disabled, frail and elderly, those at risk of functional decline, as well as mentally impaired adults age 18 and over, that require assistance and rehabilitation to remain in the community. Basic services available include: individual needs assessment; nursing care; rehabilitative therapies; social services; psychiatric/psychological services; family/individual consultation; transportation; meals, snacks and nutritional counseling; and recreational activities. Provides field trips, catered meals, physician on premises weekly. Diverse cultural multilingual staff. Cachement Area – East County: East of the Conejo Grade including Simi Valley, Thousand Oaks, Moorpark and Westlake Village. Licensed by the State of California Department of Aging.

Fees: Service reimbursable through payments from Medi-Cal or private payments. Private pay is on a non-Medi-Cal fee schedule.

Millennium Care Adult Day Health Care Center 2150 Winifred Street Simi Valley, CA 805-583-0859

Description: 16,000 square-foot facility opened March 2003. Center activities include physical and occupational therapy, and nursing services provided. Transportation, hot breakfast and lunch, and snacks provided by the center. Facility program includes games and recreational activities, and medical and social services. Serves Simi Valley and Moorpark Thousand Oaks, Westlake and Agoura Hills residents. Open Monday through Friday. Olena Samusenko, Administrator.

Fees: Private Pay - $69 per day. All fees covered by Medi-Cal for those who are eligible.

Thousand Oaks

Senior Concerns Adult Day Center 401 Hodencamp Road Thousand Oaks, CA 91360 (805) 497-0189

Description: Adult Day and Special Care programs for individuals who are isolated due to age, physical or memory impairment. Participants must be over 60 years of age and in need of supportive services, yet do not require skilled nursing care. Adult Day program is for persons with physical frailty and/or moderate memory impairment, including those with Alzheimer’s disease or other related . Special Care program is for individuals who have more severe physical impairment and/or more advanced dementia. Both programs operate Monday through Friday from 9:00 a.m. to 3:00 p.m. Early morning and late afternoon hours and two half-day Saturdays a month are available on a contract basis. Programs provide individualized care designed to maximize functional skills, foster social interaction and increase self-esteem. Each participant receives a written services plan and appraisal that is updated as needed. Senior Concerns is the only Alzheimer’s Day Care Resource Center in Ventura County. Services are provided through a partnership between Ventura County Area Agency on Aging, the California Department of Aging and Senior Concerns. Fee: Senior Concerns is a 501-3 ( C ) non-profit agency. All fees are based upon a sliding scale (participant’s ability to pay). Financial documentation is required for sliding scale consideration.

Ventura

Description: Center activities include physical and occupational therapy, and nursing services provided. Transportation, hot breakfast and lunch, and snacks provided by the center. Facility program includes ESL classes, games and recreational activities, and medical and social services. Open Monday through Friday. Serves Ventura and North Oxnard, Sylmar, Camarillo, Santa Paula, Fillmore, Peru and Ojai.

Fees: Arrangements for payment range from a sliding scale, based upon ability to pay, to individual private medical insurance plans. All fees covered by Medi-Cal.

Ages: Age 60 and older. Cost: No fee. Other: Office hours – Monday and Tuesday, 9 a.m. – 12 p.m.
