Community Tab Frequently Asked Questions

The Community tab will be displayed if your patient currently has an open referral to Community Healthcare (LCH). This FAQ aims to explain in simple language what each piece of patient data means to assist Health and Social Care professionals dealing with the patient.

Explanation of Data Items

Referral A request for a care service to be provided to a patient by an LCH service.

Care Plan A plan of treatment or health care to be provided to a patient by an LCH service.

Explanation of Data Items Deliverable in January 2017

A contact made with a patient for the delivery of healthcare. A contact may be “face to face” i.e. person to Contact person or “non-face to face” e.g. by telephone.

Medications The medication or drugs prescribed during the course of treatment provided by an LCH clinician.

Allergies Allergies as registered by LCH clinicians.

Clinical presentation as recorded by an LCH clinician following assessment and treatment which may include Observations visual presentations and limitations, measurements and patient behaviour.

The provision of an adequate level of immunisation to an individual against a specific disease as recorded on Immunisations the Community Information System.
