Recently, the Iowa Board of Medicine took the following actions: Douglas T. Cody, M.D., a 47-year-old Iowa-licensed physician who practices otolaryngology in Clinton, Iowa, had the terms of his probation terminated by the Board on March 9, 2012. On November 10, 2005, the Board charged Dr. Cody with engaging in professional incompetency and practice harmful or detrimental to the public. A hearing was held and on January 28, 2008, the Board issued a Final Decision. The Board concluded that Dr. Cody closed his medical practice without providing proper notice to patients, failed to provide appropriate treatment to patients and engaged in a pattern of disruptive behavior. Dr. Cody’s Iowa medical license was suspended, however the suspension was stayed and he was required to complete a Board-approved disruptive physician evaluation, submit a practice improvement plan and complete a Board-approved record keeping course. The Board issued Dr. Cody a public reprimand and ordered him to pay a $5,000 fine. The Board also placed Dr. Cody on probation subject to Board monitoring for a period of five years. Recently, Dr. Cody requested termination of the terms of his probation. On March 1, 2012, after careful consideration, the Board voted to terminate the terms of Dr. Cody’s probation. David K. Jones, M.D., a 45-year-old Iowa-licensed physician who practices internal medicine in Des Moines, Iowa, had the terms of his probation terminated by the Board on March 9, 2012. On April 29, 2005, Dr. Jones entered into a Physician Health Contract with the Iowa Physician Health Program (IPHP) and agreed to be monitored for mental health and substance abuse. On September 13, 2007, the Board charged Dr. Jones with violating the terms of his Physician Health Contract. On November 8, 2007, Dr. Jones entered into a Settlement Agreement with the Board to resolve the pending charges. The Board issued Dr. Jones a public reprimand and ordered him to a pay a $1,000 fine. The Board also placed Dr. Jones on probation for a period of five years subject to substance abuse and mental health monitoring. On November 13, 2008, the Board charged Dr. Jones with violating the terms of his Board Order. On April 16, 2009, Dr. Jones entered into a Settlement Agreement with the Board to resolve the pending charges. The Board issued Dr. Jones a public reprimand and ordered him to pay a $1,000 fine. The Board also ordered Dr. Jones to comply with the terms of probation established in the November 8, 2007, Settlement Agreement. Recently, Dr. Jones requested early termination of the terms of probation. On March 1, 2012, after careful consideration, the Board voted to terminate the terms of Dr. Jones’ probation. David K. Greg M. Singer, D.O., a 50-year-old Iowa-licensed physician who formerly practiced family medicine in Moline, Illinois, had the suspension of his Iowa medical license terminated by the Board on March 13, 2012. On July 14, 2011, the Board suspended Dr. Singer’s Iowa medical license for failure to pay a debt owed to the Iowa Department of Revenue. On March 13, 2012, the Board received notice from the Iowa Department of Revenue indicating that Dr. Singer has addressed the unpaid debt and the Board terminated the suspension of his Iowa medical license.
