This paper aims to describe different aspects of critical care in the FTA. For this, we reviewed official 25 information about population size and intensive care unit (ICU) bed capacity in the FTA and literature on FTA ICU specificities. Persons living in or visiting the FTA are exposed to specific risks, mainly severe road traffic 26 injuries, envenoming, stab or ballistic wounds, and emergent tropical infectious diseases. These diseases may 27 require specific knowledge and critical care management. However, there are not enough ICU beds in the FTA. 28 Indeed, there are 7.2 ICU beds/100 000 population in Guadeloupe, 7.2 in Martinique, and 4.5 in French Gui- 29 ana. In addition, seriously ill patients in remote areas regularly have to be transferred, most often by helicopter, 30 resulting in a delay in admission to intensive care. -

This review of practice is mostly based on observations in the departments of Ile de France, Rhône, Puy-de-Dôme, Haute- Garonne and Alpes-Maritimes. However, the conclusions presented in this report on the concrete implementation of asylum policies have been cross-checked and triangulated with observations of these practices in other regions and are supported by findings presented in other reports – be they official or drafted by civil society organisations. -

CARTOGRAPHY AND COMPUTING APPLICATIONS FOR EPIDEMIOLOGICAL RESEARCH IN BRITTANY Louis M.F. Masse' Michel Gallou Basavanneppa Tallur Ecole Nationale de la Sante Publique 35043 Rennes Cedex France In Rennes, at the National School of Public Health, we have used maps in many studies. Sometimes these maps were used for the study of the spread of health care facilities, and of the catchment area of customers by health institutions (hospitals, out patient clinics, nursing homes for the handicapped etc...)- Other times they were used for a more classic epidemiologic analy sis of the distribution of causes of death, diseases, or of environmental factors. Usually these studies were done in Brittany, a western, celtic, province of France in which the school is located. As regards this last activity, we used maps most ext ensively for the study of brucellosis, multiple scler osis, cancer and natural irradiation. Brucellosis; the study on brucellosis dealt with the present and past morbidity of farmers and their rela tives. It was carried out on a sampling basis, in 1975 and 1976, among the 32 farms of the "Ille et Vilaine" department. Maps were used to help draw the sample of cantons ( a canton is an aggregate of 5 - 10 villages), to secure that the sample thus selected was representative of the studied area and to make the necessary replacements of areas. This use of maps was done without the help of any computer (7). 364 Multiple sclerosis; there is, so far, no specific test to ascertain the diagnosis of multiple sclerosis. Con sequently, the computation of a reliable prevalence ratio of multiple sclerosis for any area is rather difficult. -

Bouchard FAMILY STRUCTURE AND FAMILY CONSCIOUSNESS AMONG THE ARISTOCRACY IN THE NINTH TO ELEVENTH CENTURIES* There can be no question that the period from the ninth to eleventh centuries in westem Europe was one of political upheaval and change for the aristocracy. Charlemagne’s empire was invaded, fought over, divided into new kingdoms and principalities. Fief-holding, vassalage, and castles first became widespread. Even the sorts of men who wielded power changed as new lineages first of counts and then of castellans appeared and married into previously established lines1. This political change, it is generally agreed, was accompanied by some sort of change in the family structure of the aristocracy, but there has been a good deal of debate over exactly what this change entailed. In this paper, I shall reexamine the question of noble family structure in this period, trying first to define some of the parameters of the discussion and then making suggestions on the nature of the changes in family consciousness, suggestions quite different from the conclusions many have drawn in the last twenty-five years. I shall do so using concrete examples drawn from three different lineages or family groups.
