You can also download free of charge the Caser App, which, among other services, enables you to easily consult our Medical Directory. caser.es 16 INSTRUCTIONS FOR POLICY HOLDERS INSTRUCTIONS FOR ACCESSING THE SERVICE · You are required to provide identification as a CASER policyholder by presenting your membership card (for personal and non-transferable use) and identification document. · To request any service, you must present your medical card. · You can visit any CASER doctor who appears in the Catalogue of Services – Medical Directory, with freedom of choice subject to the conditions of each policy. · All tests must be prescribed by your CASER doctor on the Company’s prescriptio n cheques. Consult the list of services that require prior authorisation. · GP or paediatrician home care services can be requested from 9 am to 5 pm only when the illness prevents a visit to the doctor’s surgery. · In the event of emergency , you can call the 24-hour medical advice and emergency service on 902 190 191 . · The A.T.S. service [registered nurses] will require a prescription from a CASER doctor. · For hospital admissions, interventions and some diagnostic tests, shown as an example on the next page, prior authorisation will be required.
