Position Summary

University of Colorado Health (UCHealth) has retained Summit Talent Group to conduct a national search for the position of Director Care Management (Director) for the Southern Colorado Region (four hospitals, 570 beds, 5,400 employees). This is an exciting time to join Colorado’s largest and most innovative health care system.

Reporting to the Chief Medical Officer, Revenue Cycle and matrix to the Regional CNO for day-to-day operations, the Director leads a team of 47 staff: Two Managers (Social Work and Care Management); Care Manager RNs; Social Worker Case Managers; Clinical Social Worker Educators; Care Coordinators; and an Administrative Assistant. The Director provides operational oversight to ensure alignment with the mission, values, and objectives of the department and organization.

Critical to this role is a leader that serves as a change agent and uses evidence-based approaches to advance the care management model. Highly collaborative and interdisciplinary in approach, the Director promotes patient centered care coordination across the continuum, that is safe and reliable. Staff development & engagement, physician relationships, capacity management, community commitment (underserved population), information technology and revenue cycle are important areas of focus to facilitate care transitions.

As part of a nationally ranked academic medical center with a reputation for providing outstanding clinical care, the incumbent will work collaboratively with nursing colleagues to promote nursing science, professional nursing culture and an inclusive environment for patients, families and employees.

The ideal candidate will have earned a BSN and a Master’s Degree, eligible for a Colorado State Licensure as a Registered Nurse (RN). A minimum of five years of experience at Director/Manager level in Case Management, Care Management and/or Utilization Review in a complex health system environment. Additionally, experience with social work, post-acute care and partnering with revenue cycle. Experience in an academic medical center is highly valued.

Colorado Springs, CO Colorado Springs is located in the east central portion of the state. It is situated on Fountain Creek and is located 60 miles south of Denver. At 6,035 feet the city stands over one mile above sea level, though some areas of the city are significantly higher and lower. Colorado Springs is situated near the base of one of the most famous American mountains, Pikes Peak, rising over 8,000 feet above the city on the eastern edge of the Southern Rocky Mountains. • Directs the department's activities and resources to ensure alignment with the mission, values, and objectives of the department and organization. • Develops and implements departmental goals, plans, and standards consistent with the clinical, administrative, legal, and ethical requirements/objectives of the organization. • Directs and evaluates departmental operations, including patient care delivery, information technologies, service level determination, and complaint management, to achieve performance and quality control objectives. • Plans and monitors staffing activities, including hiring, orienting, evaluating, disciplinary actions and continuing education initiatives. • Prepares, monitors, and evaluates departmental budgets, and ensures that the department operates in compliance with allocated funding. Coordinates and directs internal/external audits.

Organizational Governance

UCHealth is governed by an 11-member fiduciary Board of Directors. UCHealth is not an agency of the state government and is not subject to administrative direction or control by the Regents of the University of Colorado (the Regents) and is not financially accountable to the Regents, although the State of Colorado reports UCHA as a component unit. UCHealth Pikes Peak Regional Hospital (PPRH) has 15 available beds and is located just outside Woodland Park, Colorado. Wearable technology allows us to monitor blood pressure, pulse, respiration, and oxygen level on a continuous basis without disturbing patients.

LPH is a new, 51-bed community medical center providing a wide range of treatments, surgeries, and diagnostic tests for the communities of Longmont, Firestone, Frederick, Dacono, Erie and Mead. UCHealth LPH is a Level III trauma center, able to care for serious emergency and traumatic injuries.

UCHealth Grandview Hospital (GVH) in Northern Colorado Springs opened in 2017 as a state-of-the-art hospital caring for patients from minor injuries to complex medical conditions. Information Systems Highlights: Epic electronic medical record (EMR), both inpatient and outpatient, has been installed system wide. The Enterprise Resource Planning system, Lawson HRIS system and KRONOS have also been installed.
