The requirements are as follows:

1. Applications will not be accepted after the deadline. Incomplete applications will be considered late, and will not be accepted.

2. All scholarships require current full-time school enrollment. For undergraduates, full-time is 12 units or more per semester; for graduates, it is 8 units or more per semester.

3. For scholarships spread over two semesters, you must be enrolled full-time during both semesters for the 2015-16 academic year.

4. You cannot defer a scholarship’s effective dates from one semester to another. Eligibility for some scholarships may require the completion of specific classes.

5. Federal law requires that all scholarship money will be considered part of your financial aid package. It may affect your loan eligibility. Check with the Financial Aid Office before or after the scholarship is awarded if you need clarification.

6. Recipients of scholarship awards are required to prepare thank-you materials for the donor(s). Students who do not meet these requirements may have their scholarship award revoked. Instructions on thank-you expressions to donors will be forthcoming after award notifications have been sent. Thank-you materials will be shared with the donor(s) on your behalf.

If any of these stipulations are not met, the scholarship may be voided and the remaining money returned to the scholarship fund. You may also be liable for repaying monies you have already received if they were fraudulently awarded.

If you are in agreement with these terms, please sign below. If you receive a scholarship, a signed copy of this agreement will be returned to you for your records.
