P R O G R A M E L I G I B I L I T Y 1 P o s t R N B S c  Three years Diploma in General Nursing (RN) From any institute N u r s i n g T w o recognized by Pakistan Nursing Council (PNC) .  Minimum Three years Practical experience as a registered Nurse. Y e a r s P r o g r a m  One year specialization course  Valid Pakistan Nursing Council (PNC) License.  Domicile and PRC of Sindh Province. Both Males and Females are eligible to apply. 2 O n e Y e a r B S c N  Diploma in RN and RM with minimum of 3 years of clinical experience F a s t T r a c k  DWA and DTA with minimum 5 years teaching experience after DTA and DWA P r o g r a m  Valid / Active PNC Licence  Age limit is maximum 45 years  Female male ratio = 95.5 3 F o u r y e a r B S c  Matriculation , from province of sindh N u r s i n g  Minimum 50% Marks.  Intermediate Science (Pre-Medical Group ), from province of sindh ( G e n e r i c ) Both males and Females are eligible to apply. DOCUMENTS REQUIRED WITH APPLICATION FORM Photocopies of the following documents should be submitted with the application form, attested by a Government officer in grade 17 or above.

I. Matric Certificate, from province of sindh. II. Matric mark sheet III. Diploma of General Nursing (three years course). IV. One year specialization course certificate V. Mark Sheets (three years) of diploma. VI. Domicile Certificate of the applicant from Province of Sindh. VII. Permanent Resident Certificate (form-C) of the applicant of sindh province. VIII. Domicile Certificate of father from Province of Sindh. The domicile Certificate of mother shall only be accepted in case where father has expired and no domicile certificate was issued to him OR there has been remarriage/separation for which a valid document from the judge of the high court has been produced. Domicile certificate of the candidate can also be accepted if he/she has attained the age of 21 years or both the parents are not alive (statement supported by death certificate) and no domicile certificate was issued to them. In case of married female candidates the domicile of the husband will be acceptable provided the domicile of the husband and PRC of the candidate belong to the district in the province of Sindh. Any candidate found using Domicile/PRC of more than one district/place in Pakistan shall be disqualified for admission. And the case will be referred to authorities for investigations. IX. The candidates national Identity Card (or B form if he/she is below the age of eighteen years). X. Six passport size photographs with names, 3attested from front and 3attested from back. XI. Entrance test fee Payorder of Rs. 1000/- (One thousand only) in favor of Dow University of Health Sciences. BSc NURSING FAST TRACK PROGRAM (One Year)

DOCUMENTS REQUIRED Photocopies of the following documents should be submitted with the application form attested by government officer in grade 17 or above. a) Matric Certificate of sindh province b) Matric mark sheet or IBCC equivalence certificate c) Diploma of General Nursing (three years course) d) Mark sheets (three years) of Diploma. e) Diploma of DWA & DTA (PG) f) Mark sheets of Diplomas DWA & DTA g) Sindh Domicile Certificate of the applicant. h) Permanent Resident Certificate (form-C) of the applicant of sindh province in accordance with Law/Rules. i) Domicile Certificate of father, from province of sindh (any district) except employees of federal government / armed force personnel. j) The domicile Certificate of mother shall only be accepted in case where father has expired and no domicile certificate was issued to him OR there has been remarriage/separation for which a valid document from the judge of the high court has been produced.

. Application forms can be obtained from 18th June 2012, from DUHS webstie: www.duhs.edu.pk . Application forms should be submitted alongwith a payorder of Rs. 1000/- in favour of DUHS and all required documents of eligibility in the same UBL branch. . Last date for submission of application form is 20th July 2012 . Entrance Test will be held on Sunday 5th August 2012 (Venue & time will be mentioned on admit card). . Candidates if government servants should apply through proper channel with an advance copy to the Admission Cell, DUHS. . No TA/DA will be paid neither accommodation will be provided to any candidate. . The applications received after the last date will NOT be accepted. .
