THURSTON HIGH SCHOOL ATHLETIC RESPONSIBILITY ACKNOWLEDGEMENT

Prior to participating in any practice or tryout sessions for any interscholastic sport, each athlete must:

1. Successfully pass a physical examination by a registered physician and the copy of such examination must be on file with the building athletic director. One current physical examination per year is sufficient for all sports during the year. Physical must be dated as of April 15th of the current school year. (Example: 2006/2007 school year: physical must be dated after April 15, 2006)

2. Return to his/her coach (in each sport) the South Redford Schools Athletic Responsibility Acknowledgement form (properly signed.)

3. Return to his/her coach (in each sport) an acknowledgement form (properly signed) containing the significant team rules. It is understood that coaches who do not have printed team rules have no rules other than those printed below and those established by the school’s Athletic Handbook, the Michigan High School Athletic Association, Inc., the Student Code of Conduct, and the Student Handbook.

As a South Redford School’s student athlete participating voluntarily in interscholastic athletics, I understand that:

1. I will abide by the South Redford Schools Student Handbook, Student Code of Conduct, the school’s Athletic Handbook, the coach’s team rules (where applicable), and the laws of the State of Michigan.

2. I will conduct myself in a reasonable, responsible manner that is in accordance with the South Redford Code of Conduct.

3. I will be responsible for all athletic equipment issued to me throughout the season, will return such equipment at the conclusion of the season, and will pay the current replacement cost for any of the equipment not returned by me at the end of the season.

4. I will not use or be in possession of tobacco, alcohol or drugs. If I do use any of these substances, am in possession of such substances, or am suspended from school for use or possession of these substances, I understand that I will lose the privilege of competing in interscholastic athletics for a minimum period of two weeks from the time the violation occurred. If the violation occurs at the end of a season or during the student’s off-season, the date of suspension for competing in interscholastic athletics will begin from the date of the first interscholastic contest of the athlete’s next sport season.

5. I will be dropped from the team and forfeit any award should I be suspended from school for gross misbehavior, arson, assault, theft, vandalism, possession and/or using weapons.

6. If I am suspended from school during the season for reasons which do not cause me to be dropped from the team, I will not be allowed to practice or play during the duration of the school suspension.

7. I understand that the minimum penalties cited above apply during the entire school year.

I have read and understand the Student Code of Conduct, the Athletic Handbook, the coach’s team rules (where applicable) and the Student Handbook. I understand that should I violate any of the above rules, the Athletic Handbook rules, the Student Code of Conduct rules, the team rules (where applicable), or the laws of the State of Michigan, I am subject to immediate suspension from athletics and may not receive an award for the sport.

I know what is expected of an athlete who represents South Redford Schools and am prepared to so distinguish myself.

I am participating in athletics voluntarily with the understanding that I have never received money or merchandise in any amount or any emblematic award worth more than twenty-five dollars ($25.00), for participating in athletic events, and that I have never competed under an assumed name. After I have practiced with or participate in a contest with my school in any sport, I will not compete in any outside athletic contest in this sport until after the school season has been completed.

I understand that the South Redford schools do not assume financial responsibility for medical, hospital or ambulance expenses incurred because of athletic injuries, and that participation in a sport may be dangerous and students who participate my risk physical injury.
