Please carefully read the following and initial where indicated. Bitches in season will not be allowed to participate in class while in heat and credit will not be given for any classes missed. INITIAL ______Aggressive dogs are prohibited from all programs, except the Feisty Fido Program. Definition of aggressive dog is – any dog that shows an inappropriate challenge, threat or contest to another being (human or canine). Please consult with Jan Hicks if your dog has exhibited aggression in the past before applying for this class. Persons asked to leave a class because of their dog’s aggressive behavior, will not receive a refund of any remaining classes. However, a pro rata credit may be applied for private sessions. Most classes are 45 minutes long and 8 weeks in duration, unless otherwise specified.

Please refer to the Upcoming Class Schedule or Which Class is for You? for class fees of the class for which you are enrolling. Make checks payable to Paw Law Dog Training Academy. Since registration is limited, no refunds will be given after the first class. Applications must be returned as soon as possible to ensure placement in the class, as enrollment is limited for many classes. Applications received without payment will not guarantee a placement in the class.

Classes will be held at Paw Law Dog Training Academy’s facility at 1053 Lake St., (Lake St. about ½ mile west of Gary Ave.; and ½ mile east of the Elgin/O’Hare Expressway ---NE corner of Lake St. & Bartels, across the street from McDonald’s and next door to Car Quest Auto Parts.)

Please fill out the application and return with enrollment fee (made payable to PAW LAW DOG TRAINING ACADEMY) to: Janice Hicks, 33 Gromer Road, Elgin, IL 60120 (847-888-4725 or 630-893-5789). There will be a $25.00 charge for checks that are returned NSF. What to bring to class: 1. ONE roll of paper towels to be kept by Paw Law Dog Training Academy for use during class sessions. 2. Copy of current health certificate for Paw Law Dog Training Academy to keep for its records from your vet showing proof of vaccinations and negative fecal. Due to insurance requirements, we must have proof of vaccinations before your dog can attend class. No health records means NO class. 3. Questions and specific training/behavior problems you would like addressed in class. 4. For classes with your dog, your dog on a short (4’ to 6’) leash, no retractable (Flexis) will be allowed. 5. Waiver, Assumption of Risk and Agreement to Hold Harmless

In consideration of an internship/externship, and/or paid or complimentary dog training lessons and/or doggy daycare, I hereby waive and release Jan Hicks and/or Don Ward d/b/a Paw Law Dog Training Academy & Daycare, their employees, volunteers, officers, directors, and representatives as well as property owners from any and all liability of any nature, for injury or damage which I or if applicable, my dog may suffer, including specifically, but not without limitation, any injury or damage resulting from the action of any dog or other person; and I expressly assume the risk of any such damage or injury while attending any function, training, or doggy daycare provided by Paw Law Dog Training Academy & Daycare or while on the property, training grounds or surrounding area thereto; (1053 Lake, Hanover Park, IL and/or 33 Gromer Rd., Elgin, IL.

In consideration of and as inducement to the acceptance of my participation in any event, training, or doggy daycare, I hereby agree to indemnify and hold harmless Jan Hicks, Don Ward and Paw Law Dog Training Academy & Daycare, their employees, volunteers officers, directors, and representatives as well as property owners from any and all claims, or claims by any member of my family or any other person accompanying me to any event, training, doggy daycare, or function of Paw Law Dog Training & Daycare Academy, while on the grounds or the surrounding property thereto as a result of any action by any dog, including my own, or any person.

I fully understand that attendance at a dog event, function, training or doggy daycare is not without risk to myself, members of my family or guests who may accompany me, or if applicable to my dog, because some of the dogs to which I (we) will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest of care. I fully understanding these risks associated with dog events, dog training and doggy daycare, I agree to hold harmless Jan Hicks, Don Ward and Paw Law Dog Training Academy & Daycare in the event that any dog causes injury to myself, or in the event that my dog causes injury to myself or any other person or dog, I will reimburse person or dog’s owner immediately after receiving the bill.
