Extract No. The extract number corresponds to the number of the batch obtained from the splitting of the original consignment (e.g.: No. of the certificate of inspection is 9994. The batches of this consignment are given the numbers 9994-1, 9994-2 etc.)
Box 2: This box indicates the legal bases which are relevant for the issue and use of this ex- tract; indicate the relevant provision, namely Article 23 (list of countries), Article 23a (list of recognized certification bodies and inspection authorities) or Article 24 (individu- al authorisation) of the Organic Farming Ordinance.
Box 3: The serial number of the underlying certification of inspection which was given by the issuing body or authority.
Box 4: The authorisation number in case of import under Article 24 (individual authorisation). This box is completed by the issuing body or, when the information is not yet available at the time the issuing body endorses box 15, by the first consignee or the importer.
Box 6: Inspection body or authority in charge of inspection of the operator having split the consignment.
Box 14: Shall be completed by the competent certification body for each of the batches result- ing from the splitting operation referred to in Article 16f.
Box 15: Shall be filled in at the reception of the batch, when the consignee has carried out the checks provided for in Annex 1 point 8.5 of the Organic Farming Ordinance.
