ARIZONA CARDINALS SKILLS & EDUCATIONAL CAMP

The Arizona Cardinals Football Club is hosting a FREE Football Skills & Educational Camp presented by Dignity Health and SRP, on Saturday, May 19 th , 2012 for high school football players at University of Phoenix Stadium Lawn C (1 Cardinals Dr, Glendale, AZ 85305). The athletes will have the opportunity to receive on-field instruction from the Pros, as well as classroom discussions, study hall, do’s & don’ts, life skills and health & nutrition. The following session will be offered:

Skills Camp Schedule Check-In: 8:00 AM-8:30 AM Classroom (Parents and Coaches Invited):8:30 AM-9:30 AM Transition to Field: 9:30 AM-9:45 AM Stretching: 9:45 AM-10:15 AM On Field: 10:15 AM-12:15 PM Combine Conditioning: 12:15 PM-1:15 PM Lunch/End Camp: 1:15 PM

All registrants must submit this registration form by the deadline in order to receive lunch and a shirt. Space is limited. You will be notified if you are put on the camp waiting list. It is imperative that you complete and return the attached form to: Arizona Cardinals Football Skills & Educational Camp (P.O. Box 888, Phoenix, AZ 85001) or fax the registration to (480) 785-7327. All participants may only register and participate in one group. Forms with 2 positions specified will NOT be accepted. All entries must be received by May 6 th , 2012. I authorize my child to participate in the Arizona Cardinals Skills and Educational Camp on 5/19/2012 (the “Event”). I understand that there are risks and dangers involved in the Event, and I voluntarily assume all of those risks and dangers, including the risks and dangers of personal injury and property damage that may occur during or arise from the Event. In exchange for being permitted to participate in the Event, I hereby expressly and unequivocally release and discharge the National Football League, its thirty-two (32) member professional football teams, NFL Properties LLC, NFL Ventures L.P., SRP, Dignity Health and their respective officers, directors, employees and volunteers, subcontractors, sponsors, agents, affiliates, subsidiaries and shareholders (collectively the “Releasees”) from all present and future claims that may be made by me, my family, estate, heirs or assigns for property damage, personal injury, or wrongful death arising as a result of my participation in the Event wherever, whenever or however the same may occur. I understand the Event may be photographed, videotaped or otherwise recorded, and I agree to let the Releasees use my name, likeness and demographic information free of charge in any manner and for any purpose in any media now known or hereafter developed.

I am not under any legal disabilities that prevent me from being legally bound by this Waiver and Release of Liability for Personal Injury and Property Damage (this “Release”). I agree that this Release, and any dispute arising from or involving this Release, shall be governed by and construed in accordance with the laws of the State of Arizona.
