Academic Clinical Fellow in Special Care Dentistry: Health Education West Midlands Training Programme in Special Care Dentistry

Training placements to be based in the Health Education West Midlands / Birmingham Community Healthcare Trust area

SUMMARY

This is a non-resident Academic Clinical Fellow (ACF) position in Special Care Dentistry (SCD) based in the Birmingham Community Healthcare Trust (BCHC), which also covers part of the Black Country adjacent to Birmingham, and the University Of Birmingham School Of Dentistry.

The training programme has been approved by the Postgraduate Dental Dean for higher training in the specialty of Special Care Dentistry, and offers very wide experience. The Postgraduate Dental Dean has confirmed that these placements have the required education and staffing approval.

The training content will reflect the Special Care Dentistry competencies as laid down by the Specialty Advisory Committee (SAC) in SCD.

The programme provides for the ACF to spend time in Community settings, the Birmingham Dental Hospital and General Hospitals over the course of the training programme. The ACF may be seconded to other approved Community Trusts and other organisations to broaden experience and fulfil specific educational requirements. Placement of an individual trainee within Primary Care will be dependent upon an assessment of the trainee's previous experience and future training needs and will be planned to ensure that a trainee gains the widest possible experience of Primary care work.

APPOINTMENT DETAIL

1. The Post - Academic Clinical Fellowship Programme in Special Care Dentistry

This programme integrates academic and clinical training in Special Care Dentistry providing a supervised programme which will allow candidates to undertake research training in the School of Dentistry and to complete clinical training in BCHC leading to a CCST in Special Care Dentistry.

Clinical and academic training 25% of clinical training will be protected for academic pursuits. During dedicated research time they will develop preliminary results for use in a fellowship application, which they will

1 submit in year 3.

The School of Dentistry is part of the College of Medical and Dental Sciences and has a strong Postgraduate ethos. There will be a supportive network guiding the ACF in their training. The ACF will be provided with a clinical educational supervisor and a research supervisor who will direct their training and help them put together an application for a fellowship. In addition they will be allocated an academic mentor who will provide objective, confidential and informed career advice. The ACF will also be able to take advantage of the research training offered by the University. This will include developing their own training needs analysis.

2. Duration of post The post is for a minimum period of 3 years. It is expected that the ACF will successfully compete for external funding towards PhD training at the end of 3 years. Prior experience will be taken into account and the length of training will be determined by competence assessment (ARCP).

3. Clinical - bases for training Community dental clinics throughout Birmingham and the Black Country (Walsall, Sandwell and Dudley), Psychiatric Secure Units, Birmingham Dental Hospital, and General Hospital / other locations as required.

5. Arrangements for protected research time The protected research time within this programme will be on a session release basis. This will include regular timetabled sessions week by week, along with specific modules of academic development training on a block basis.

6. Research component of programme  This may include fields such as dementia, mental health, sedation, medically compromised patients, learning disabilities, or an area of the candidate’s own interest if appropriate. A cardiac / sedation research trial has recently obtained ethical approval, and candidates would be most welcome to participate in this.  The School of Dentistry has excellent research facilities together with a number of fruitful research developments across the College of Medical and Dental Sciences.  The successful applicant will be encouraged to work collaboratively with colleagues in other research streams to develop ideas and protocols. An academic supervisor from within the School of Dentistry will work with the trainee and supervise their development as well as their progress towards grant submission.

7. Clinical training component of programme

Clinical training will take place predominantly within the Birmingham Dental Hospital. The Dental Hospital is an integral part of the Birmingham Community Health Trust providing secondary and tertiary care services to a regional population in excess of 6 million people.

The successful applicant will work alongside 2 full-time NHS StRs in Special Care Dentistry. This programme is designed to fulfil the SAC curriculum leading to a CCST in Special Care Dentistry.

3 OUTLINE DESCRIPTION OF THE TRAINING PROGRAMME

Entry requirements

Candidates for the placement are required to hold full registration with the General Dental Council, and have completed general professional / foundation training. Experience in the provision of primary care dentistry in the UK is essential. The applicant will also need to be eligible for a PCT performer number with BCHC.

Possession of a higher qualification in SCD approved by the SAC in Special Care Dentistry would be advantageous. Wide experience in dentistry is also desirable.

Duration of the training programme

Skills to be developed during the training programme

Aim of the Training Programme.

The training programme will ensure that, at completion, the trainee has sufficient:

1) Knowledge, both academic and practical 2) Skills (both clinical and academic, e.g. pt care, presentations) 3) Opportunity to develop attitudes conducive to providing SCD care 4) Experience of all aspects of SCD care delivery 5) And undertaken a substantive research project

to enable him or her to successfully complete training including the ARCP process, undertake the Intercollegiate Tricollegiate examination, obtain a Certificate of Completion of Specialty Training, and to be entered on the Specialty List in Special Care Dentistry.

The detailed training programme will be designed to be appropriate to the educational needs of the person appointed to the placement. It will therefore take account of the extent and depth of relevant experience already obtained by the trainee; however the

4 trainee will still have to provide evidence through the use of Workplace Based Assessments (WBA) to demonstrate that they have the required competencies in the areas where they are considered to have prior experience.

In order to achieve this objective, the training programme will provide the experience and the educational support to ensure that the ACF has developed an appropriate level of knowledge, judgment, skills and attitudes that are required to be a Specialist in Special Care Dentistry as set out in the Specialty Training Curriculum.

Content of the training programme

The training programme will commence with a period of induction and orientation; during which time the extent and depth of existing relevant knowledge and experience of the trainee will be identified through joint discussion between trainers and trainee in order to tailor the content of the training programme to the needs of the individual.

The trainee will then undertake a variety of projects relevant to his or her training needs and appropriate to the work of the training organizations. An example from our current StR programme is close involvement in a regional care home survey involving homes in several Strategic Health Authorities, from initial planning through to field work and collation / analysis of results. These will be undertaken with a degree of supervision appropriate to the learning status of the trainee, within the general approach that supervision will be more direct in the early stages of the training programme and diminish as the trainee develops.

The trainee will also undertake the formal academic component of the training programme. This will mainly be undertaken during the first part of the training programme so that training will normally remain to be completed after the successful acquisition of an approved postgraduate degree.

Degrees such as a Masters in Special Care Dentistry (from King’s College London and University College London), the Membership in Special Care Dentistry (from the RCS Edinburgh) and the Diploma in Special Care Dentistry (from RCS England) are approved by the SAC in Special Care Dentistry for formal academic training in the specialty.

Learning Outcomes of the Training Programme.

After successful completion of the training programme, the trainee will:

 Be able to demonstrate an understanding of the basics of biological science in order that she/he can recognise normal and disordered growth and development and its implications for oral/dental care

 Have the knowledge, understanding, attitudes and skills to provide appropriate care for people who have a physical, sensory, intellectual, mental, medical, emotional or

5 social impairment or disability and be competent to undertake the diagnosis, treatment planning and treatment pertaining to their Speciality care.

 Be competent to plan and provide a high quality of oral care to people with disabilities and complex needs through a comprehensive understanding of disability issues.

 Be able to take account of the individual’s social, physical and cognitive circumstances to establish optimal oral care plans.

 Have a comprehensive understanding of disability issues and the impact that they can have on oral health; as well as a comprehensive understanding of the impact that oral health can have on the lives of people with disabilities and complex needs.

 Have developed skills in the management and delivery of oral health care, and the development of oral health promotion programmes for a range of special groups of people so that she/he is competent in the design, implementation and completion of preventive oral health care programmes for people who have a physical, sensory, intellectual, mental, medical, emotional or social impairment or disability.

 Have a thorough understanding of behavioural sciences relating to people who have a physical, sensory, intellectual, mental, medical, emotional or social impairment or disability.

 Be experienced in behaviour management techniques required to provide oral care for people who have a physical, sensory, intellectual, mental, medical, emotional or social impairment or disability (including the appropriate use of local anaesthesia, conscious sedation and general anaesthesia) and be able to provide comprehensive oral care using the most appropriate treatment modality

 Have the ability to confer with, and refer to, other oral health care professionals and to communicate findings and treatment proposals to individuals, carers and other professionals.

 Be able to collaborate in multi-professional teams for the management of people with disabilities

 Be able to diagnose, and initiate emergency treatment of, commonly occurring medical emergencies.

 Have undertaken a substantive research project, with a view to possible progression to undertaking a PhD after completion of the ACF training.

 Be trained in appropriate scientific methodology and research techniques and have prepared articles suitable for publication

 Have developed teaching and communication skills

 Be competent to take the lead in, implement and carry out clinical audit and other aspects of clinical governance

6  Understand the legal and ethical framework within which dentists practice and, particularly, those issues pertinent to Special Care Dentistry.

Clinical Work Programme

Except when undertaking formal academic training and / or research, the trainee will normally spend time each week within CCDS clinics. Time utilization in a particular week will be dependent upon the particular projects in hand at that time and agreed with the Training Programme Director.

For clinical work, the organisation has 9 multi surgery clinics in (often recently built) multi-professional health centres, with full clinical facilities including piped IHS gases and fully equipped suites for IV sedation. A full range of domiciliary equipment is available. The ACF will have access to all aspects of clinical training as detailed within the SAC training documents.

The ACF will also have training in working inside psychiatric secure units, and will be expected to take a lead role in providing clinical services using the newly built dental clinic within Yardley Green secure unit in the later part of their training. Experience in prison dentistry can also be provided if desired by the ACF.

Currently there is a 1 session / week for 6 months attachment in Oral Medicine at BDH, and this will be expanded to include an attachment in the Restorative Dept at BDH. The ACF will also spend 1 session / week for 6 months in the Oral and Maxillofacial Dept. at the Queen Elizabeth hospital undertaking surgical dentistry procedures. Input into treatment sessions under general anaesthesia are also timetabled.

Additional specialist services / clinics can will be accessed within the Birmingham Dental Hospital, the newly built Queen Elizabeth Hospital in Birmingham, and other outlying hospitals in Birmingham, Dudley, Sandwell and Walsall – in order to ensure satisfactory completion of the programme.

Programme management and administration

The programme will be jointly managed by the School of Dentistry, Birmingham Community Healthcare Trust and the local training committee of the Health Education West Midlands Deanery. Appointees will be assessed through the Annual Review of Competence Progression (ACRP) process. The committee will not only assess the trainees but also ensure that the training programme is meeting its objectives.

Supervision of training

Progress in training will be regularly reviewed by discussion between the trainee and

7 responsible consultants and through more formal appraisal reviews at three-monthly intervals. There will be regular assessment of achievement of the outcomes of training. The trainee will be able to approach the Training Programme Director in SCD and their Educational Supervisor as required and vice versa outside of the regular assessments.

Annual reviews will take place prior to the annual review of competence progression (ARCP). In addition, progress in training and the operation of the training programme will be subject to regular reporting and review in the Health Education England West Midland Training Committee, chaired by the Training Programme Director. This in turn reports to the Dental Training Programme Directors meeting chaired by the Associate Postgraduate Dental Dean for Specialty training

Completion of training

Successful completion of training would involve satisfactory progress through Annual Review of Competency Progression (ARCP), completion of WBA, portfolios etc, and the trainee presenting for and passing the Tricollegiate Membership in Special Care Dentistryexit examination.

FACILITIES AVAILABLE TO THE Academic Clinical Fellow

A trainee's main base will initially be the Aston or Stockland Green dental clinics. The trainee will have full office facilities, including secretarial support from the SCD team administrator, and networked personal computing facilities.

The library facilities in the BDH and the University of Birmingham will be available to the trainee. Office accommodation and appropriate IT access will also be available at Birmingham Dental Hospital to support academic sessions.

. AUDIT ARRANGEMENTS

The trainee and consultant trainers will participate in formal audit arrangements. In addition, the opportunity (if available) will be taken to join with other ACFs / StRs and other colleagues in relevant audit activities in the neighbouring Community Trusts..

PERIOD OF NOTICE

The placements are subject to a minimum period of notice on either side of three

8 months. Progress in the post will be subject to regular review. Failure of the trainee to satisfactorily complete approved academic training or achieve a satisfactory ARCP outcome could lead to extension or possibly termination of training in accordance with teh Dental Gold Guide.

TRAVELLING

Travelling will be required in these placements both elsewhere in the Region and further afield. Possession of a current driving license is therefore highly desirable. Alternative arrangements to comply with the Disability Discrimination Act (DDA) will be made if necessary.

Mileages will be calculated from the designated main base for the purpose of reimbursement of traveling expenses.

STUDY LEAVE

Study leave will be granted within recommended levels* for relevant courses and conferences subject to the prior formal approval of the employing authority in each case. This will include (where necessary) leave with pay and appropriate expenses to attend an approved Masters / Membership Degree or Diploma course Special Care Dentistry.
