21.1 GENERAL

21.1.1 The Director of Maintenance is responsible for the airworthiness of all Company Aviation Services, LLC aircraft, engines, rotors, accessories and appliances installed on the aircraft. He may delegate the functions of the operations to the other maintenance personnel, but retains the responsibility for all inspections and maintenance.

21.1.2 Inspections of the aircraft, engines, rotors, accessories and appliances will be performed by FAA approved repair stations or appropriately certificated airmen. All maintenance personnel that work on Company Aviation Services, LLC aircraft will be covered under an FAA approved Drug Program. Inspection and maintenance of the aircraft engines, rotors, accessories and appliances, when the aircraft are located at the home base, will be conducted by Company Aviation Services, LLC maintenance personnel.

21.1.3 All maintenance, preventive maintenance and alteration to the aircraft, engines, rotors, accessories and appliances will be performed in accordance with current FAA regulations, manufacturer’s service manuals’ recommendations and specifications, manufacturer’s service letters, airworthiness directives and good maintenance practices. Following any maintenance, appropriate entries will be made in aircraft and engine logbooks, flight log books and other company required maintenance records prior to flight. All life-limited parts that have reached the mandatory replacement limit (hours, cycles or calendar) will be properly disposed of in order to ensure the part will not be re-used after having reached its mandatory life limit.

21.1.4 An operational flight check will be made following any maintenance operation that could change the flight characteristics of the aircraft. The Director of Maintenance will ensure that all special inspections and AD notes are complied with as required.

21.1.5 The DOM will review all revisions to the Manufacturer’s Maintenance Program and submit a copy of those revisions to the Pagosa FSDO for approval. Upon approval, the revisions will be incorporated into Company Aviation Services, LLC maintenance program and applicable manuals. All repairs, alterations, major or minor, will be recorded in the permanent maintenance records of the equipment in accordance with the requirements of FAR 43.9.

21.1.6 The Pilot in Command shall make a record of all mechanical irregularities or defect that comes to his attention before, during and at the completion of the flight, as appropriate, in the aircraft flight log book. He shall submit the log to the Chief Pilot or the Director of Maintenance.

21.1.7 The Pilot in Command assigned for flight, shall review the aircraft log book prior to flight, for disposition of discrepancies. The aircraft log book shall be kept in the aircraft during flight and while away from the home base.

21.2 INSPECTIONS

21.2.1 All Company Aviation Services, LLC aircraft will be inspected in accordance with the approved Manufacturer’s maintenance program and in accordance with part 135.419 of the Federal Aviation Regulations the Operations Specifications.

21.2.2 All maintenance entries which are required to be made in the Aircraft Log Book will only be made by personnel authorized by the Director of Maintenance. All aircraft will be maintained using the Manufacturer’s Recommended Maintenance Program.

21.2.3 Compliance with Airworthiness Directives will be recorded in the AD record section and the Aircraft/Engine Log Book. Each applicable AD will be identified by number, the date and aircraft/engine total time, description of how it was complied with and the signature and mechanic certificate number of the person responsible for its completion. When a new Airworthiness Directive that applies to any of Company Aviation Services, LLC aircraft, is received, the Director of Maintenance will review it carefully and confer with the Director of Operations concerning aircraft operating limitations that may be affected.

21.2.4 All ADs which have recurring maintenance will be listed on the Maintenance Due Log Located in the back of the Aircraft Log Book by maintenance personnel. The Director of Maintenance will update the Helicopter data sheet with the new Airworthiness Directives Compliance due date/hours, and indicate that the aircraft is now “in service” in the Company Aviation Services, LLC aircraft status board.

21.3 PARTS ORDERING PROCEDURES

21.3.1 Aircraft parts and maintenance supplies are to be purchased only by authorization of the Director of Maintenance.

21.3.2 The Director of Maintenance is to establish parts and supplies procurement procedures which will ensure the least possible delay in accomplishing the required maintenance of Company aircraft.

21.4 SERVICE DIFFICULTY REPORTS (OPERATIONAL) 135.415

21.4.1 The Director of Maintenance is responsible to see that all Service Difficulty Reports (SDR) are completed and delivered as required below.

21.4.2 The Director of Maintenance shall send each SDR required, in writing, covering each twenty-four (24) period, beginning at 0900 hours local time of each day and ending at 0900 hours local time on the next day to the Pagosa FAA Flight Standards District Office. Each report of occurrences during a twenty-for (24) period will submitted to the FAA within the ninety-six (96) hours. However, a report that is due on Saturday or Sunday may be submitted on the following Monday, and a report due on a holiday may be submitted on the next workday. For aircraft operated in area where mail is not collected, reports may be mailed or delivered within twenty-four (24) hours after the aircraft returns to a point where the mail is collected. The report shall include as much of those items that are available listed in 135.415(e) using FAA Form 8010-4 “Malfunction or Defect Report”.

21.4.3 The Director of Maintenance shall report the occurrence of detection of each failure, malfunction or defect on an aircraft concerning:

(a) Any fire and, when monitored by a related fire-warning system, whether the fire- warning system functioned properly;

(b) Any false warning of fire or smoke;

(c) An engine exhaust system that causes damage to the engine, adjacent structure, equipment, or components;

(d) An aircraft component that causes the accumulation or circulation of smoke, vapor, or toxic or noxious fumes;

(e) Any engine flameout or shutdown during flight or ground operations;

(f) A propeller feathering system or ability of the system to control overspeed;

(g) A fuel or fuel-dumping system that affects fuel flow or causes hazardous leakage;

(h) A landing gear extension or retraction, or the opening or closing of landing gear doors during flight;

(i) Any brake system component that results in any detectable loss of brake actuating force when the aircraft is in motion on the ground;

(j) Any aircraft component or system that results in a rejected take-off after initiation of the take-off roll or the taking of emergency action, as defined by the Aircraft Flight Manual or Pilot’s Operating Handbook;

21.4.3 The Director of Maintenance shall report the occurrence of detection of each failure, malfunction or defect on an aircraft concerning, Continued…

(k) Any emergency evacuation system or component including any exit door, passenger emergency evacuation lighting system, or evacuation equipment found to be defective, or that fails to perform the intended function during an actual emergency or during training, testing, maintenance, demonstrations, or inadvertent deployments; and

(l) Auto-throttle, auto-flight, or flight control systems or components of these systems.

21.5 MECHANICAL INTERRUPTION SUMMARY REPORT 135.417

21.5.1 The Director of Operations shall submit to the Pagosa Flight Standards District Office, before the end of the 10th day of the following month, a summary report for the previous month of each interruption to a flight, unscheduled change of aircraft enroute, unscheduled engine stop or diversion from a route, or unscheduled engine removal caused by known or suspected mechanical difficulties or malfunctions that are not required to be reported under Parts 135.415 and 416.

21.6 SUSPECTED UNAPPROVED PARTS PROGRAM

21.6.1 Company Aviation Services, LLC will follow the guidance outlined in AC 21-29B (chg 2) for the monitoring of suspected unapproved parts. Should an unapproved part be discovered, a FAA Form 8120-11 (dated 6/2001) will be filled out per the instructions outlined in AC21028B and mailed to the following address:

21.7 OBTAINING MAINTENANCE AWAY FROM HOME BASE [135.23(h)]

21.7.1 If the aircraft requires preventative maintenance, maintenance, or servicing while away from the home base, PIC will contact the Director of Maintenance or his designee for instructions. If this is not possible, the PIC is authorized to contract for minor maintenance items up to $500 (US). In the event the required repairs will exceed this amount, the PIC will contact the Director of Operations or the Chief Pilot. Unless the PIC is also a holder of a mechanical certificate issued by the FAA, he will not perform maintenance or preventative maintenance on any of Company Aviation Services, LLC aircraft.

21.7.2 The Pilot in Command is responsible for determining whether the aircraft is in condition for safe flight. He will not operate the aircraft unless it is in airworthy condition and, if any airworthy mechanical or structural condition should occur during flight, he will comply with the emergency as described in the manufacturer flight manual. If a condition permits, the Pilot in Command shall land at the nearest airport or maintenance facility where repairs can be performed.

21.7.3 Non Company Aviation Services, LLC maintenance technicians will be permitted to accomplish “away from home” maintenance providing they have been approved by the Director of Maintenance. The DOM is responsible for determining if the technicians and facilities are qualified in accordance with FAA regulations and the requirements of this manual and the Manufacturer’s Recommended Maintenance Procedures. The PIC will ensure all maintenance is monitored by a flight crewmember.

21.7.4 Aircraft parts and maintenance supplies are to be purchased only by authorization of the Director of Maintenance. The PIC is responsible for returning any invoices, billing copies, warranties, or other paperwork for maintenance performed away from home the Director of Maintenance. If the DOM is not available the Director of Operations will insure these requirements are met. Both Officers may direct the PIC to verify that all requirements are met.

21.7.5 No changes or alterations of any type will be made to Company Aviation Services, LLC aircraft without prior approval of the Director of Maintenance.

21.7.6 The Director of Maintenance will perform an inspection of all “away from home” maintenance as soon as the aircraft returns to it’s home base.

21.7.7 It is the responsibility of the PIC and the DOM to determine that the technician is properly certified and makes the proper entries in the maintenance logbooks for his work when repairs and maintenance are conducted away from the home base.

21.7.8 No maintenance technician will be permitted to perform maintenance repair work on Company Aviation Services, LLC’s aircraft, unless he or she is enrolled in an FAA approved Drug testing program.
