REQUEST FOR PROPOSAL FOR THE PROCUREMENT OF AN HEALTH MAINTENANCE ORGANIZATION (HMO)
The Association for Reproductive and Family Health (ARFH) has received grants from Global Fund for AIDS, Tuberculosis and Malaria towards implementing the TB/HIV New Funding Model (NFM) Project in Nigeria and intend to apply part of the proceeds of the grant to procure a Health Maintenance Organization (HMO) to provide Health Insurance and health care benefit plans for staff across the country. ARFH now invites two (2) separately sealed Technical and Financial bids.
Project: Annual Hiring of an HMO to provide Health care Coverage for staff of ARFH.
Pre-qualification: Your company must be a registered HMO organization with the requisite experience and track records for provision of Health Care/Insurance. The documents below must be made available in a sealed Technical bid (the scoring for each is stated and will be used as such in the selection); Please Note that the Financial bid will not be opened if the score for the Pre-qualification is below 70% score):
Minimum Requirements:
. Evidence of company’s incorporation with Corporate Affairs Commission (CAC)
. Evidence of three years (2014 – 2016) Tax Clearance Certificate
. Evidence of a Tax Identification Number (TIN)
Technnical Requirements:
. Company Profile with in-country staff strength and registration with relevant Government Regulatory and professional bodies; proof of attendance to any of the any HMO local and International conferences and workshop and will enable the vendor to attract maximum points – 15%
. Evidence of having functional Branches/presence in at least 24 States plus FCT in the Country including FCT (Please state verifiable addresses and contact persons) – 25%
. Evidence of existing contracts with at least 2 International Organizations, 5 Non- governmental Organizations and 5 Privately/Public owned organizations in Nigeria (All the organizations must have at least 50 staff strength) – 25%
. Evidence of affiliation/registration with at least 5 Prime/Teaching Hospital; 7 Secondary Health Facilities and 10 Specialist Clinics/Hospitals across the 24 States – 25%
. Evidence of recent (2012 – 2014) annual Returns and current audited accounts of your firm for the past three years (2012 – 2014) – 10% Financial Bid/Price Quotes: All Vendors are expected to submit all their Health Care Plans in a Tabular format using Spreadsheets with the detailed services available for each plan and the cadre of Hospitals available for it with the unit cost per staff (Add soft copies in a flash drive). Note that the hard copy of the financial bid will have to accompany your submission to ensure committee members endorse their signatures on it after bid opening. For all the proposals meeting technical requirements, the lowest financial compliant proposal shall be selected after the financial bid analysis. All unit costs quoted for each of the cost line on the cost template below provided shall be evaluated and selected for each of the Health care Provider cadres activities and shall be strictly based on the lowest cost. Therefore, vendors should ensure that all their line unit costs are competitive. For all the proposals meeting technical requirements, the lowest financial compliant proposal shall be selected after the financial bid analysis.

. All bid document submitted for the purpose of this procurement process shall be duly signed by each of the Procurement committee member present on the Bid Opening day for proper validation of the documents.
. Your quotation should reach the undersigned within 30 days from the date of publishing of this advert.
. This request for proposals solicitation has been advertised in numerous print and electronic media and hence, will be very competitive.
. The quotations shall be valid and not subject to review for the one year period from the date of contract signing by the selected Third Party Logistics Companies.
. The successful bidder(s) shall be notified in writing and will be invited to sign a contract with ARFH.
. Contract period is January 2018 to December 2018.
Method of Submission: Original copy of the 2 bids should reach the address as stated below on or before 30days of publishing this call for proposals, sealed separately in an envelope marked on the top right side with ‘HMO Pro – Technical Bid’ and ‘HMO Pro – Financial Bid’ and addressed to:
