In discussion with you we can also recommend any changes to ______EHC Plan that might be needed. We will discuss and agree proposed changes to the EHC Plan.

Please return the slip below to confirm you will attend.

Reports from the school and other professionals will be sent to you at least two weeks before the meeting. So you can have the opportunity to think about your views and feedback before the meeting.

You and ______may also send us your own reports if you wish. Your contributions to this review are very important. Some ideas about what you might put in the report are enclosed.

Please find enclosed a leaflet called Your Child, Your Family Your Annual Review, what to expect at an Annual Review. Let me know if you need more information about the Annual Review process.

You are welcome to bring a friend or an advisor to the meeting.

GUIDANCE NOTES

1. Invite letters These are example letters that you can use to send to parents and professionals to invite them to the meeting

2. Parents report for Annual Review Please encourage parents to provide their views for the review. This is a template that you can send with the invite letter

3. Child or Young Persons View Parents and our educational setting can support the child or Young person to gain their views of the last year and the coming year. This can be adapted to the child/young person completing the form but we do require the information for the review. You may find it helpful to look at the Buckinghamshire County Council’s, Easy Guide to Transition found on the local offer about support young person and gain their views.

4. Setting Report for Annual Review for Education, Health and Care Plan The education setting MUST complete a report before the annual review. This can be the basis of the Annual Review.
