3x SKILLS SUPPORT WORKERS

GRADE: 5 (SCP 19-25) SALARY: £18,746 - £22,658 CONTRACT – Permanent, 37hours Monday to Friday

We are looking for a dependable, dedicated, responsible, caring person to join our enthusiastic and motivated Day Opportunities Team.

You will be required to work with adults with disabilities and life limiting conditions to achieve person centred outcomes whilst promoting their independence, equality, dignity, individuality and quality of life. Our Day Opportunities Team is looking for a hard working and dedicated person who can use their excellent communication skills to develop person-centred care plans in partnership with people. For the role of skills support assistant we are looking for people with the right attitude and the desire to make a difference in peoples lives. You will be confident working on your own initiative and as part of a team and have the ability to provide sensitive personal care when needed. The job will provide you with an opportunity to gain skills, knowledge, training and experience in supporting people to access community-based activities in individual and group work in a variety of settings. This will include individuals with complex and/or behavioural needs. It is desirable that you hold a valid UK drivers licence along with GCSE (or equivalent including pre- assessment) English and Maths and have or be willing to obtain the Qualification Credit Frame work in Health and Social Care (Adults) level 2 for Wales and NI OR equivalent qualification/experience in a related field. Job Purpose

To work with adults with disabilities and life limiting conditions to achieve person centred outcomes. To promote the independence, equality, dignity, individuality, quality of life and choice for all people in receipt of services.

Principal Accountabilities and Responsibilities

1. To work together with others (across teams and communities) to deliver quality opportunities which promote positive outcomes for vulnerable adults recognising the need for positive risk management. 2. To understand the needs and requirements of people with learning disabilities, autism, mental health issues, dementia, behaviours that challenge, physical disabilities, sensory loss, acquired brain injury and other conditions. 3. To implement person centred approaches that support each individual’s personal development To promote and actively encourage service user and carer involvement in all aspects of work particularly in the development of person centred plans 4. To provide support to a variety of therapeutic interventions as required. 5. To provide support to individuals in order to maintain and develop skills for life (including work). 6. To facilitate learning and development activities that are person centred and meet individual needs and preferences. 7. To support and assist service users to build up local networks in their community, making connections and assisting them to gain new interests and experiences. 8. To contribute to person centred assessment, planning, implementation and review. 9. To support individuals with assistive technology. 10.To promote positive behaviour and to implement the positive behavioural support Model 11.To take on the keyworker role as required 12.To participate in reviews, assessments and risk assessment processes 13.To assist with travel and travel arrangements 14.To implement best practice such as person centred communication. Developing a range of communication styles and effective listening skills. 15.To support the recording, monitoring and evaluation of service users progress ensuring that this is consistent with legislation, guidance and policy and procedure. 16.To ensure the promotion of equality, inclusion and co production in all aspects of work 17.To understand and implement risk management tools. 18.To participate fully in the monthly supervision and annual appraisal process 19.To undertake any training as required and to develop all relevant work skills 20.To implement best practice in manual handling, personal care and hygiene. Moving and handling people in line with their individual plans of care 21.To comply with the Health and Safety at Work Act 1974 and all other health and safety legislation. To comply with TCBC policy and procedure at all times. 22.Any other duties commensurate with the post as directed by the Principal Officer for Service Provision and Development

7. Demonstrate good timekeeping and attendance completing any agreed paperwork to the required standards and on time.
