Position: Support Worker / Project Co-ordinator Salary: £18,500 - £21,500 per annum Responsible to: Service Manager/ Executive Director

JOB SUMMARY

1. To help promote a stimulating, caring and culturally appropriate environment for the Service Users. 2. To provide practical and emotional support to Service Users, under the guidance of the Management team. 3. To work closely with Service Users and their families according to programmes and plans that have been implemented. 4. To deputise in the absence of the service manager. 5. To lead shifts when most senior person on duty is unavailable or when directed to do so by the service manager.

MAIN RESPONSIBILITIES AND DUTIES:

General Duties:

Supervising of Support Workers and Volunteers.

a. Being responsible for anticipating Service User’s needs and communicating these needs to other staff and professionals involved. b. Writing review reports and other reports as necessary. c. Being involved in review meetings concerning Service Users. d. Being involved in setting up and implementing personal care plans for Service Users. e. To be involved in the selection, assessment, and admission of new Service Users with the service manager. f. To help new Service Users settle into the home. g. Ensuring hand over sheets are up to date and complete.

To participate in the full range of caring duties including:

a. Working closely with SIAS, Solihull DIP, Prison in-reach team and all other referring agencies, to assess service users for the house and to refer service users on to relevant support services and move on accommodation. b. To co-ordinate volunteers, to enable smooth running of the house and to organize sleep in rota’s and cover any shortfall. c. To facilitate groups and one to one’s with service users to discuss relevant issues regarding their recovery. d. To signpost service users to relevant agencies to ensure a comprehensive, individual support package is being provided. e. Helping Service Users with skill development inside and outside the home. f. Listening to Service Users, including their feelings about any difficulties they may be having. . g. Helping Service Users to make informed choices. h. Implementing personal care plans set out for Service Users. i. Communicating with other staff/volunteers about Service Users’ needs/activities. . j. Facilitating monthly Service User meetings. k. To help ensure that each Service User’s health needs are met including: - Liaising with medical services (e.g. GP, Dentist)

Team a. To participate in staff and team meetings, training courses and supervision sessions. b. To be a constructive member of the staff team. c. Work co-operatively with fellow workers and to use own initiative.

Other Duties a. To undertake any delegated tasks from the service manager such as weekly checks and monitoring b. To support the service manager during recruitment and induction programmes for peer mentors c. To act in accordance with all policies, e.g. Health and Safety, Admission

HOURS AND ANNUAL LEAVE A flexible 37.5 hours a week is worked which may include evenings and weekends. Holiday entitlement: 28 days holiday per year plus statutory Bank Holidays.

PLACE OF WORK Clarity House, Chelmsley Wood and other Changes UK properties if required.

Educational Standards Basic Literacy Skills

Professional Qualifications NVQ Level 3 in Health & Social Care NVQ Level 4 in Health & Social Care

In-Service Training Commitment to attend training and Health and Safety/ First Aid/ Certificate team meetings, as required. Food Hygiene/ Moving & Handling

Relevant Job Experience Previous experience in working with Previous experience of project drug/alcohol abuse. management.
