FBI Background Check Information needed for Chile Student Visa

1. You must have an FBI background check to submit with your student visa application. Due to the time it takes to process the FBI background check, you must submit proof of your application in order to be considered for acceptance to the Chile program.

2. Visit the FBI background information website: http://www.fbi.gov/hq/cjisd/fprequest.htm Scroll down to how to request a copy of record.

3. Click on complete cover letter. Fill in the appropriate information. Your full name. The address where you live. Fill in your name after I. List the address you want the background check mailed to. In the section I have a reason/date that requires expeditious handling enter December 15, 2010 for the date required. For reason list: I am applying for a student visa to study in Chile for the spring 2011 semester. An FBI background check is required. It takes several weeks for the Consulate to process the student visa. Enter your phone number and email. Print the letter out and sign it. You must also PLACE DATE/REASON FOR EXPEDITING ON OUTSIDE OF MAILING ENVELOPE. Submit a photocopy of this letter to IAP with your application materials to Bascom 250.

4. IAP will provide a letter to support your reasons for expediting the background check. This will be available in 250 Bascom Hall; you will receive an email notification when they are ready for pick up.

5. Obtain set of fingerprints with your name, date of birth and place of birth. They have a downloadable standard fingerprint form on the FBI webpage. UW-Madison police department (http://www.uwpd.wisc.edu/Fingerprinting.html) offers fingerprinting 3 days a week (Tuesday, Wednesday and Thursday from 8:15 a.m. -3:30 p.m. Cost per fingerprint card is $10. 6. Include $18 money order or certified check payable to the Treasury of the United States.

7. Mail items 3, 4, 5 and 6 above to: FBI CJIS Division - Records Request 1000 Custer Hollow Road Clarksburg, West Virginia 26306 These items should be mailed to the FBI no later than March 15, 2011.
