Shantel’s Wagging Tails Client Agreement I request that my dog(s) be accepted into Shantel’s Wagging Tails Doggy Day Care, boarding or other programs In accepting my dog(s) into the programs, I acknowledge that the Facility will be relying on my understanding and agreement with the provisions of this Agreement. I understand and agree that:

Every dog reacts differently and animals, by nature, are unpredictable and, therefore, present a certain level of risk. I acknowledge and understand there are certain risks involved in participating in day care or boarding and in owning, training, caring for, handling and being in the presence of dogs, including but not limited to injuries to people, property and animals resulting from dog fights, dog bites to humans or other dogs and the transmission of disease.

In case of emergency or for the use of the Canine Cab services, I recognize the risks of injury that accompany said transport and acknowledge that this Agreement is being relied upon by this Facility to permit transport of my dog to and from the Facility or any other necessary location. I accept any and all conditions, rules and regulations promulgated by this Facility associated with the activities, use of the facilities and transport, and hereby agree to comply with them.

Dogs not regularly exposed to the level of activity at the Facility may feel the discomfort of sore muscles, sore joints and fatigue, and dogs not regularly exposed to outside activity or play on hard surfaces may experience sore paws, blisters, bruises or abrasions on the feet (Raw Paw).

Dogs not regularly socialized do not necessarily know how to behave politely with other dogs. These dogs are at higher risk of incidents including, but not limited to, bites, fights, fear aggression, object guarding, behavior problems and/or acting out.

Dogs at play during day care get dirty. Dogs with longer hair can get matted from the level of activity at day care. Brush outs can be requested and scheduled. Excessively long toenails may cause injury. I will be notified if my dog’s nails are too long.

Water is available at all times; however, my dog may still be thirsty after day care. I will be aware of their water intake as excessive amounts may cause an upset stomach or other problems.

Dramatic changes in food and/or food quality may cause upset stomachs, diarrhea, and/or colitis.

Any behavior deemed dangerous or inappropriate by Facility personnel may result in dismissal of my dog(s) from the programs. I must pay for all services at the time they are rendered. I understand any unpaid fees by me will be sent to collections, and I will be responsible for all collections and legal fees incurred by such actions taken.

In admitting my dog(s) into the Facility, the Facility has relied on my representation that my dog(s) is/are in good health, has/have not been ill with any communicable disease within the last 30 days and has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog.

I have read and understand the Facility’s “Rules and Regulations.”

I understand that Facility staff gives all pets involved in any type of incident a cursory examination; however, the Facility’s staff are not trained veterinarians and will not be liable for the location, treatment or diagnosis of any injuries incurred on the Facility’s premises. If my dog(s) is/are involved in an incident at the Facility, I will check my dog(s) further or seek treatment for my dog(s) by a licensed veterinarian at my discretion and cost.

I hereby release Shantel’s wagging Tails from any and all liability should my dog(s) suffer injury, illness, loss or death. I understand that under no circumstance will Shantel’s Wagging Tails be liable for my damages.

Photographs, videotape or digital recordings are taken of the facility, dogs, customers and staff on a regular basis for, among other things, use in advertising. I acknowledge that all such images (electronic, negatives and positives), together with prints and the copyrights therein, are the property of Shantel’s Wagging Tails. I give Shantel’s Wagging Tails my consent, permission and authorization, without compensation to me, to use, reproduce and alter the images, in print or electronic format (including on the internet), either alone or in combination with other images, text and graphics. I waive my right to approve the finished photograph, advertising copy, print material or electronic files that may be used in conjunction with the images. I represent that I am at least 18 years of age and acknowledge that the rights granted in this paragraph are irrevocable, worldwide and perpetual, and are binding on my heirs, successors and assigns.

I grant the Facility and/or its agent’s full power of decision concerning the care and well being of my dog(s). Should any medical emergency arise, it is agreed that the Facility or its selected agents can and will make any needed decision concerning medical treatment and choice of care giver up to $______, and I will be responsible for reimbursing all such costs to the Facility.

What you should know when starting Daycare or Boarding EXHAUSTION –

Especially after the first few days of daycare for a new dog, you are going to notice a huge difference in them when they go home. They will literally sleep for days. This is completely normal! Being in a new place your dogs mind isn’t going to stop, smelling new things, meeting new dogs and the excitement of the day will really knock a dog out.

SORE MUSCLES –

Dogs just want to have fun, and meet the other pups that come to play. Most dogs go crazy every time they see or hear someone new, they just get so excited. None of the dogs want to miss out on the fun; unfortunately some dogs forget to pace themselves! Expect sore pups to go home with you for the first couple of days. When you first start daycare we suggest skipping a day here and there to allow your pup time to relax at least until they become accustomed to their new lifestyle.

WEIGHT LOSS –

If up until now, exercise for your dog meant a half hour walk around the block or an hour at the dog park expect major weight loss if you do not adjust their feeding. Dogs that go to daycare every day during the week should be considered an active dog on most dog food bags. Adjust their feeding to avoid unwanted weight loss.

WET COAT –

Your dog may be wet when you pick them up at the end of the day, this could be because of playing in the snow or if it started to rain.

FLY BITES –

Your dog can get fly bites just like we can. They can be mistaken for an allergic reaction, so please be aware.

STRESS –

Dogs can become stressed in many different scenarios. Daycare is more unlikely to cause your pup stress, but for some it’s just too much. In these cases, we suggest trying half days at daycare and slowly working your way up to a full day. For boarding, we always recommend bring your pup by for a few daycare visits before a long stay. This will help your dog understand that he is not being left, this is a happy and fun place to be, and it also gives them a chance to become accustom to the new sounds and smells. Prepare your dog before long stays and they will have no trouble staying with us at all. If while you are gone your pup shows signs of stress, you will be notified ASAP.

DIARRHEA –

Stress is the main factor for a dog to get diarrhea, however another major factor can be food change. Please make certain you supply enough food when leaving you pup for a long period.
