6.1 Need for the study Pama is a Kapha Pitta1 predominant disorder presenting in Ayurvedic clinical practice, which may be considered as “Scabies” in contemporary science. It is highly infectious and may spreads in a group of population within short period, so its control leads to prophylaxis towards the further spread of the disease.

The incidence of this disease is very high in children and young adult age group.

This disease is caused by an Arthropoda called “Sarcoptes Scabie”.

Khadirashtaka Kwatha contains the drugs Khadira,Triphala, Nimba, Guduchi, Vasa and Patola which are having Kustaghna and Kandughna properties because of Tikta Rasa predominance. Hence in Yogaratnakara it is indicated for the treatment of all varieties of

Kusta5.

Chakradatta has advised Durvadya Taila for Abhyanga in the treatment of

Vicharchika, Kacchu and Pama6.

As both External and Internal medications are mostly useful in the treatment of skin disorders. The present study is included to observe the role of Khadirashtaka Kwatha internally and Durvadya Taila externally in the management of Pama Kusta

6.2 Review of literature

 Review of literature related to Pama-Kusta as per Bhruhatrayi, Laghutrayi and other

textbooks of Ayurveda.

 Review of literature in detail related to Scabies as per modern texts.

 Review of the ingredients of Khadirashtaka Kwatha and Durvadya Taila including the

properties and actions of Yogas as per Yogaratnakara and Chakradatta

 Collections of related references from available sources like Journals, Internet regarding the

disease and drug combinations. Previous work done

6.3 Objectives of the study

 To study the efficacy of Khadirashtaka Kwatha internally and Durvadya Taila externally in

the management of Pama Kusta.

 To study in detail about Pama Kusta as per Ayurveda.

 To study about Scabies as per modern science.

6.4 Hypothesis

Ho: There is no significant effect of Durvadya Taila and Khadirashtaka Kwatha in the

Management of Pama

HA: There is significant effect of both Durvadya taila and Khadirashtaka Kwatha in

Pama

HB: There is significant effect of Durvadya Taila in Pama 7. MATERIALS AND METHODS

7.1 SOURCE OF DATA

Children attending to Koumarabhritya OPD and IPD of Alva’s Ayurveda Medical

College and Hospital Moodbidri, within the age group of 6-12 years will be selected for the proposed study irrespective of their sex, caste, religion and socio-economic status.

7.2 METHOD OF COLLECTION OF DATA

Minimum 40 diagnosed children with Pama Kusta will be randomly selected for the clinical study with pre-test and post-test design and will be divided into 2 groups of 20 children each.

A) Procedure of administration of drug

Group A- Patients will be treated with internal administration of Khadirashtaka Kwatha in the dose of 5-10 ml twice before food diluting by adding water accordingly and Durvadya Taila Abhyanga is advised once daily before bath. Group B- Patients will be treated with only Durvadya Taila Abhyanga as advised in Group A. Treatment will be continued for a period of 28 days or until satisfactory result is obtained which ever comes earlier.

B) Observation period

Clinical response will be assessed on 7th, 14th, 21st, 28th day

C) Inclusion criteria

 Children diagnosed as the case of Pama Kusta /scabies with the presenting symptoms.

 Children in between the age group of 6-12 years.

 Children of either sex irrespective of caste, religion and socio-economic status. D) Exclusion criteria

 Children below the age of 6 years and above 12 years.

 Kusta other than the Pama Kusta.

 Children suffering from any other systemic disorders like Tuberculosis, HIV, Leprosy etc.

 Scabies leading to complications such as Eczematization, Secondary streptococcal infection,

Urticaria etc.

E) Diagnostic criteria

Diagnosis will be made based upon signs and symptoms of Pama Kusta /Scabies such as

 Thoda (Pain)

 Kandu (Itching)

 Daha (Burning sensation)

 Pitika(Burrows)

 Srava(Discharge)

F) Assessment criteria

Detailed proforma will be prepared for the assessment of effect of the ingredients in both the groups by grading subjective and objective parameters. The data obtained will be assessed statistically by adopting appropriate tests 7.3. Does the study require any investigations or interventions to be conducted on patients

or humans or animals? If so, please describe briefly: -

Yes. The clinical study will be conducted in the OPD patients of Alva’s Ayurveda Medical College Hospital. No animal experiment will be carried out. Laboratory investigations Blood –Hb%,TC,DC.ESR. Microscopic examination of burrows (if necessary).

7.4. Has ethical clearance been obtained from your institution in case 7.3? Yes, clearance has been obtained through the ethical committee formulated in the Institution.
