PCAC Quality Subcommittee Conference Meeting April 2, 2014

April 2, 2014 at 12:00 PM pacific/2:00 PM central/3:00 PM eastern 60 minute conference.

Our hope is to be able to have you all give one another a glimpse of what you are using as a spiritual assessment in your EMR, knowing EPIC is what most of you are using while two of you use Meditech and one Cerner).

Our ultimate goal is to work with the Quality Subcommittee of the Pastoral Care Advisory Committee of CHA to see if we can: 1. determine what are core elements of our electronic spiritual assessments that can be seen as “leading practice” for this work 2. explore what kind of reports, and potential quality measures, that data from these assessments can provide.

I think I recall seeing in the recent Vision that there was a request for ideas on how systems are using EPIC. Attached is our Spiritual Assessment “Smart Text.” It doesn’t actually look like this in the chart—only the drop down selections which are selected actually appear. It has been well received. We also have customized the census screen to show the date of the last visit, Communion, date patient was anointed, and any other special issues at a glance (ie NO VISITS or SPANISH ONLY). I created this when OSF Hospice went onto EPIC last year as what they had for hospice was unacceptable to me. Let me know the best way to do this. Thanks!

Carolanne Hauck Lancaster General Health, Lancaster PA, Hi David, we are using EPIC here at Lancaster General Health and are finding it to be a good way to document our contribution to the Plan of Care. We have made our own Chaplain’s report and make use of a template for writing notes on the Plan of Care. We are also using the Clinical Practice Guidelines (CPGs) and finding that to be a good tool too. Let me know if you need anything else. I am glad that we are working on this together. Mary Toole, St. Francis Hospital, Roslyn, NY, part of CHSLI Dear David, I am a chaplain at St. Francis Hospital, Roslyn, NY, part of the Catholic Health System of Long Island (CHSLI). Our hospital started using EPIC 12/1/2012. I was designated the super user for our Pastoral Care department which meant I needed to assist all members of the department in learning/using the electronic system. If you have not received volunteers from other members of CHSLI and if I could be of service, I am interested in being of assistance.

Matthew.Kronberg, DignityHealth Part of a Dignity Health team that just updated our Cerner Spiritual Care Assessment. While we just finished a substantial overhaul, we are already looking to the future and would appreciate hearing more about what others are doing.

Janice.Labas-Lucht and Richard Brochu Part of CHE Trinity –Use Meditech and have created a variety of reports from the system.

Method Each of the six takes 5-6 minutes to share. Then 25-30 minutes for question, observations.

Other questions for consideration

When we try to measure quality, and we examine our electronic spiritual assessments and our reports, what might be elements that could become part of quality measures?

Could there be any consensus a out leading practices with spiritual assessment in EMR?

From the quality measures based on existing role of chaplains – when considering outpatient settings, what might be track for determining what kind of quality measures to attain in the outpatient world – MD’s for referrals Building community partnerships More asset development along with distress assessment/management
