2011-2012 Palmetto Dance Center’s Studio Policies This form must be signed by parent or legal Guardian prior to student’s first class. Your signature on the registration form indicates you have read, understood, and agree to comply with these policies. Release of Liability I will not hold any member of Palmetto Dance Center responsible for any accidents or injuries occurring to (name of child or self if over 18)______during practice or traveling to & from practice, competitions or performances. Studio Policies (please check each box to indicate you have read, understand and agree to comply with our policies.) o This registration form also serves as a press release and indicates your agreement with our use of you/your child’s image in advertising and marketing of Palmetto Dance Center. o Tuition is charged on a monthly basis and is determined by the number of classes a student takes per week and how many students from the same family are in classes. o Tuition payments are not refundable and are not discounted for missed classes, holidays or classes cancelled due to inclement weather. Students will be charged a full month of tuition regardless of attendance. o 30-days written notification will be required for withdrawal from the program. Tuition will be charged if proper notification is not given. o Tuition payments are due the 5th of each month and are considered late after the 10th. Tuition that is late will be charged a $10 late fee. o Returned checks will be charged a $10 returned check fee. o There are no refunds for unused tuition or recital, costume/shoe deposits. o Tuition can be paid by monthly check or auto payment drafted from your debit or credit card. If you pay for a semester or the entire year, a discount will be given. o Costume fee is due by October 27th. Fees are $60 for all Combo I and Combo II Classes and $75 for all other classes. o Students with delinquent accounts will not be allowed to participate in the Recital. o Every student participating in the recital must pay a $40 recital fee and will receive 2 complimentary tickets to the recital. o Photos will be made in the spring. Dancers are not obligated to purchase any photos but are asked to be present for group photo. o If a dancer or parent needs to speak with a teacher, they are asked to leave a number where they can be reached during the day, and the teacher will call you as soon as possible. Time taken between classes delays the start of the next class and affects every class for the remainder of the day. o Palmetto Dance Center reserves the right to terminate the dance instruction of any student at any time, with or without cause. o The Studio generally follows the schedule for Kershaw County School district. We will not close for teacher workdays. If schools are closed for bad weather, please check studio voicemail or website by noon that day to see if classes will be held. o Palmetto Dance Center is not and shall not be responsible for injuries received by any student while attending class or to any person while on the premises of Palmetto Dance Center. o Parents of small children are required to be at the premises when their child’s class ends for the purposes of picking up their child. Palmetto Dance Center specifically is not responsible for dance students who choose not to attend class after being dropped off at Palmetto Dance Center or who choose to leave the studio premises during or following their class. o Palmetto Dance Center is not responsible for loss or theft or articles brought onto the studio premises. Classroom Etiquette o Each student should bring appropriate shoes to class. Dance Shoes are worn only inside. o Please see that your child is dressed in appropriate dancewear. NO JEANS ALLOWED! Pull hair away from face. o No Food or Chewing gum allowed. o If your child is in class for more than 45 minutes, please bring a water bottle. Come prepared to sweat. o Good attendance is imperative as absences and tardiness can hold back an entire class. o Classes progress very quickly and when a student misses several classes it is very difficult for her to stay at the same level as classmates. If 2 classes in a row are missed for any reason we request that you schedule a private session with your instructor to get caught up. o Students are asked to be ready with their dance shoes on when the teacher asks them to enter the classroom. o Disruptive students will be asked to leave the classroom. Poor behavior from a student or parent can result in dismissal from the class and the studio. o Parents, please see that young children go to the restroom BEFORE coming to class.
