Instructions: Fill out this Field History Sheet for all fields (organic, transitional, and conventional). You can use your own form as long as it contains the same information. List all inputs used or planned for use including all fertilizers, herbicides, pesticides, compost, and manure products. For 2010, list the names of all of the inputs you plan on using. If you are using a brand name product, make sure you list the brand name. For example, ‘Bt’ as a pest control is too generic a listing, ‘Dipel DF’ is the specific reference we need. For 2009, list the names of all of the products you actually used. This form must accompany your Organic Farm Plan. You do not need to list amounts or rates of material applied.
