Barbizon of Pittsburgh Release Agreement of the Passport To Discovery 2018

You must return this form, properly executed (by a parent, if under the age of 18) to Barbizon no later than September 19, 2017.

I, the undersigned, agree to abide by the following rules and regulations as set forth by Barbizon Model and Talent Management while attending and preparing for the 2018 Passport To Discovery to be held on a Royal Caribbean ship, October 1 - October 6, 2018.

1. All participants, including parents and guests, fall under the authority of Barbizon Model and Talent Management directors and management. My cooperation is required and only enhances my chances for success.

2. All payments are due on the dates indicated in my original audition package.

3. School meetings are mandatory. My commitment includes scheduled meetings. I will be held accountable for all information given at these meetings, so I will pay attention and take notes.

4. I am preparing to enter an industry in which my appearance will be constantly scrutinized. As such, I will attend all meetings neatly groomed and in appropriate attire.

5. While on the Passport To Discovery, I will be subject to a curfew.

6. I understand that my potential success and achievement at the Passport To Discovery are not guaranteed by Barbizon Model and Talent Management, and they can guarantee neither agency contracts nor “call backs”.

7. I will not consume alcohol or any controlled substance not prescribed to me by my physician.

8. I will not engage in any discussions or agreements with any agencies, photographers, etc. nor accompany them anywhere without the express permission of my Director. I will direct anyone seeking to engage me in such discussion, agreement, or contracts to meet with my Director during and after the convention.

9. I hereby release Barbizon Model and Talent Management and it’s directors from any and all liability. In consideration for the potential for publicity, I hereby grant Barbizon Model and Talent Management, its management and assigns, the right to copyright, reproduce, or otherwise use, anywhere in the world, forever, any and all photographs of me for any and all purposes including, but not limited to, the commercial use of my image for advertising, publicity, and trade (A ‘release’).

Violation of any of these, or subsequent rules established by Barbizon Model and Talent Management, is cause for me to be sent home at my own expense.

I affirm that I have read each and every word of the foregoing and will retain a copy of this release agreement.
