Education, Health and Care Plan – Annual Review Information
Dear Colleagues,
The SEN team are now able to accept Annual Reviews electronically.
The criteria to accepting a Education, Health and Care Plan Annual Review is the following :-
1. Education, Health and Care Plan Annual Reviews should be returned to The SEN Team as soon as possible after it has taken place. Preferably within 10 days as the Local Authority have to respond to parents within 28 days of the Annual Review meeting. 2. It must be emailed as a scanned document. (Do not email just the word document as this will not have been signed by the Head Teacher or the Parents / carers). 3. Section 13 must be completed if the pupil is in Year 9. 4. Section 14 must be completed. (Recommendation of the Annual Review). 5. Section 14 must be clearly completed. (Amendments required to the EHC Plan). 6. It must be signed by the relevant person i.e. The Head Teacher in Section 16. (A printed signature will not be acceptable). 7. It must be signed by the parent / carer in Section 16.
If there are any changes to the Education, Health and Care Plan a word version of these changes would be very helpful.
If you have written the changes on the previous Education, Health and Care Plan that will still be ok to be scanned and sent in electronically with the annual review.

At present if you would like to continue sending the Annual Reviews in paper format into the SEN Team then that will still be ok to do so. Obviously the cost of postage is removed for the school if you scan and then email into the SEN Team.
