August 2016 PART I Instructions: Using the Situation, Soldier Data, and Figure 1, answer questions 1 -4 regarding the award recommendation for SFC Jones.

Situation:

At 2015 on 23 February 2014, SFC Jones rescued three children from a burning building in the family housing area. The children, Jonathan Adams, age 6; Ashley Adams, age 3; and Jessica Adams, age 2, had been trapped by fire in an upstairs bedroom of their home. Their parents were not at home at the time of the fire. The babysitter, Nicole Williams, age 16, ran from the house screaming for help. SFC Jones lives next door and was alerted by a huge explosion.

The fire had spread rapidly throughout the first floor of the house when SFC Jones went inside to rescue the trapped children. He found each child and carried them to safety from the burning building. The roof of the building had collapsed by the time the fire department arrived at 2025. SFC Jones and the children were treated at Winn Army Community Hospital for second degree burns and smoke inhalation. SFC Jones’ company commander is recommending him for the Soldier’s Medal. There is no DA Form 268, Report to Suspend Favorable Personnel Actions, in SFC Jones’ MPRJ.

Situation:

SSG Branson’s supervisor and S-2 NCOIC, SFC Jeff Adams (HHC 4th AR BN), wants to recommend her for the Army Commendation Medal and have it ready for her award ceremony on 1 October 2014. SFC Adams has asked you to assist him with the award by preparing items 1- 18 of the DA Form 638. Instructions: Using the ERB provided in the Practical Exercise supplement and the additional information provided below, complete a DA Form 638 (Recommendation for Awards), Items 1-18, and route via Forms Content Management to your instructor.
