EFFECTIVE DATE: 04/01/2004 REVIEWED/REVISED: 07/10/2006, 03/03/2009, 03/30/2010, 03/24/2011, 07/11/2012, 10/14/2013, 11/7/2014, 11/11/2015 ______POLICY: PATHS is committed to providing an environment that fosters quality health care for all patients. The Patient Rights (see attachment) illustrate aspects of care which patients can expect to receive during their visit. All employees are expected to assist patients in understanding and exercising these rights.

PROCEDURE:

1. This policy will be shared, and understood, among all PATHS employees.

2. Each new employee will receive information regarding Patient’s Rights during their “New Employee Orientation.”

3. These rights and responsibilities will be posted in each practice site.

4. Patients can request a copy of the Patient Rights and Responsibilities at any time. A printable version can be found on eCliniForms, under “Other.”

Patient Rights and Responsibilities ______

Piedmont Access to Health Services, Inc. wants to give you the best health care possible. As a patient at PATHS Community Medical Centers, you may expect to receive considerate and respectful care. We will honor your rights to be informed and to be involved in making decisions about your care. As a patient, you have the following rights:

1. You have the right to know about your illness, treatment, and what might happen later. This information will be given to you by your provider in language you can understand.

2. You have the right to make decisions about your treatment. You have the right to know why you need a treatment and who will perform that treatment.

3. You have the right to refuse care or treatment and to know what may happen if you do not have this care or treatment.

4. You have the right to have all information about your illness and care treated confidential.

5. You have the right to know the name of the health care provider who is in charge of your care. You also have the right to know the names of all other clinic staff taking care of you.

6. You have the right to know about your health care provider’s certifications, years of experience, and experience in performing certain procedures. You also have the right to know your clinic’s background and experience in providing certain services, and how to resolve complaints.

7. You have the right to agree or refuse to take part in any study or experiment related to your care or treatment.

8. You have the right to receive prompt and reasonable responses to your requests for service without fear of discrimination.

9. You have the right to know what services are provided and the cost of those services. You also have the right to know the health care plans in which we participate.

10. You have the right to review your bill and ask any questions you have about it.

01-02-012 Patient Rights and Responsibilities Page 2 of 3 Health care is best when there is an open, trusting, and helpful relationship between you and the people taking care of you. We will make every effort to see that you receive the best care we can give.

In order to receive the highest quality care, you and your family are responsible for:

1. Providing accurate information about your present illness and past medical history.

2. Seeking clarification when necessary to fully understand your health problems and the proposed plan of care.

3. Following through on your agreed plan of care.

4. Considering and respecting the rights of others.

5. Being courteous to medical providers and employee staff.

6. Providing accurate information for insurance claims and working with PATHS to make payment arrangements when necessary so that others can benefit from the services provided here.

We encourage you to become involved in your health care decisions by communicating with your health care provider and learning good health habits.

We would like to know if you have any concerns about your treatment. Please discuss them with your doctor or your nurse.
