Students will be transported either by First Student Transportation or the Ebenezer Church vans. Our camp staff will accompany the students to and from the above mentioned destinations. If you wish for your child to participate in the above mentioned field trips, it is required that you complete and sign the bottom portion and return as soon as possible. By signing this form, you authorize your child’s participation on each field trip. It is imperative that we have, ON-HAND, a signed permission form for all campers prior to departure time of each field trip. Please note, parental approval may NOT be obtained via telephone.
