LONGITUDINAL INTERVENTION: NETWORKING CARE AND EDUCATING FOR DIABETES
AT THE OHIO STATE UNIVERSITY
CONSTITUTION
ARTICLE I. PURPOSE AND DEFINITION
SECTION I.1 NAME
Longitudinal intervention: networking care and educating for diabetes at The Ohio State University (hereafter referred to as LINKED)
SECTION I.2 PURPOSE
The organization is formed with the purpose of facilitating the development of The Ohio State University medical students in their knowledge and ability to link diabetic patients with primary care homes (PCH). This will include the recruitment and training of interested medical students on diabetic management and enabling them to successfully link diabetic patients to PCHs.
SECTION I.3 NON-DISCRIMINATION POLICY
This organization and its members shall not discriminate against any individual(s) for reasons of age, color, disability, gender identity or expression, national origin, race, religion, sex, sexual orientation, or veteran status.

SECTION II.1 ELIGIBILITY
All OSU Students are eligible for membership in LINKED. To maintain active membership, members must attend at least two group meetings or events per academic year.
SECTION II.2 VOTING RIGHTS
Voting membership is limited to currently enrolled Ohio State students, which are active in the group as described above. Others such as faculty, alumni, professionals, etc. are encouraged to become members but are limited to a status of non-voting honorary members.
SECTION II.3 REMOVAL OF MEMBERS
Any member who participates in any controversial or discriminatory practices while representing the organization or fail to participate in any activities throughout one calendar year, may have their membership revoked after review and a majority vote by the Executive Board.
Any member who steals or uses monetary funds from the organization’s account or special programs for any unauthorized use, will have their membership revoked immediately.
ARTICLE III. ORGANIZATION LEADERSHIP
SECTION III.1 PRESIDENT
The position of President will extend for one year. The President will preside at all regular and special meetings of the club and the Executive Board. He/She shall, with the approval of the Executive Board officers, appoint all standing and special committees and, in the event of a vacancy in the Executive Board, shall, with the approval of the board, fill such vacancy by appointment until the next regular election of the club. He shall be an ex officio member of all committees and oversee all organizational activities.
SECTION III.2 VICE PRESIDENT
The position of Vice-President will extend for one year. The Vice-President shall succeed to the office of president in the event or removal of the latter for whatever cause, and, in the absence of the president, shall preside at all general meetings of the organization and of the Executive Board.

The position of Treasurer will extend for one year. The Treasurer shall have custody of all club funds, maintaining all necessary records and depositing all such funds in a bank approved by the Executive Board. He/She shall report on the club’s financial status, including a budget describing expected fund allocation, at least once a term at the executive meeting, and shall hold all records available for inspection by any group member. The Treasurer shall ensure clear and accurate record keeping of yearly income and expenses. All disbursements shall be by check for costs incurred by members during club events.
SECTION III.4 SECRETARY
The position of Secretary will extend for one year. The Secretary will record and maintain general meeting minutes and non-financial records of the organization. He/She shall also maintain the official membership list and contact information for use by the Executive Board and officers.
SECTION III.5 CLINIC DIRECTOR
The position of Clinic Director will extend for one year. The Clinic Director shall be responsible for the relations between LINKED and all the designated clinic or hospital. He/She will also maintain all materials involved in clinics interactions ensuring clear and accurate record keeping of group and project activities.
ARTICLE IV. COMMITTEES:
Any officer may form a committee or team to aid in the business or activities of LINKED. Teams may include, but are not limited to, Fundraising Team, Clinic Team, and Publicity Team. Other non- elected committee positions may be created or assigned to assist with team operations. These positions may include but are not limited to: Fundraising Coordinator, Clinic Coordinator, Publicity Coordinator, and Special Events Coordinator.
ARTICLE V. METHOD OF SELECTING AND REMOVING OFFICERS
LINKED will conduct a general election, open to all members, once every academic year. Elections will generally be held in April to choose the leadership for the following term year. All members shall be given adequate notice of the election date and shall be informed of the results before the end of the academic year. All leadership positions shall begin and end on the final day of the academic spring term. In the event of a vacant leadership position, a by-election will be held to fill the vacant position(s).
LINKED at The Ohio State University Constitution Last Revised April 2018 Page 3 of 6 If an elected officer is considered by the general membership to not be fulfilling the duties of their office, they may be impeached. A request for impeachment must be signed by 5 active members, active members totaling one-fourth of total memberships, or a single elected officer. Upon the implementation of an impeachment process, a special meeting shall be called, with two weeks’ notice given to all members, and a vote to remove an elected officer will be taken by method of a secret ballot. The leaders not in question shall designate an unbiased member to conduct the impeachment process. A majority vote in favor of the removal of an elected officer will result in an election being called to fill the vacant position.
ARTICLE VI. ADVISORY BOARD
The Registered Advisor of the organization must be a member of the Ohio State University faculty or Administrative & Professional staff. Advisors shall be informed of the organization’s progress by the Executive Board at least once a term, in a formal meeting, and are free to attend any meetings or events. Advisors shall act in their capacity to guide the Executive Board when necessary but will not have direct authority over the board or members of the group.
ARTICLE VII. MEETINGS OF THE ORGANIZATION
At least three general on-campus meetings will be held each academic year. Minutes of each meeting will be kept by the secretary and be made available to any member that requests them. Many smaller group training meetings at clinic sites, headed by the Executive board or Clinic coordinators, will be announced throughout the academic year. Proxy votes will not be allowed at meetings.
ARTICLE VIII. METHOD OF AMENDING THE CONSTITUTION
SECTION VIII.1 BALLOT INITIATIVE
Ballot initiative is defined by an amendment, article or revision proposed to the organization in good standing. The Ballot Initiative must either fit into or add to the existing architecture of the constitution. The location of the initiative will be indicated by the author. Should the author forfeit this right, the Executive Board may decide.

A ballot initiative is proposed and subsequently tabled for at least one week before polling for adoption as law. A special meeting will be held no more than three weeks following proposition to discuss and vote upon acceptance of ballot initiative.
SECTION VIII.3 RATIFICATION OF INITIATIVE
To meet quorum for passing ballot initiatives, greater than 50% of the membership must be present at the meeting. Each member present will receive one vote. Passage of ballot initiatives requires a two-third majority vote. Should the number of adjudicators present at meeting fall below three, the Executive Board members will substitute with approval of the quorum until there are three supervisors for the poll.
SECTION VIII.4 ENTRANCE OF INITIATIVE
The Executive Board must enter the approved ballot initiative into the authoritative constitution no more than a week after passage. The updated version of the constitution must be accessible to all members within a week of passage.
ARTICLE IX. METHOD OF DISSOLUTION OF ORGANIZATION
Shall it be determined by the Executive Board that the organization is no longer needed, for reasons to include an absence of student interest, a vote of the membership shall decide if the group is dissolved. If this is not possible, a unanimous decision by the Executive board will also dissolve the organization. All financial debts shall be paid, and any remaining funds shall be donated to a non- profit community service organization on campus.
