Your web login to The Family Practice Program has been suspended because of non- payment. This could be due to the credit card info we have on file becoming inactive because of a lost or stolen card or simply because your credit card has expired. Since your agreement remains active and in force during this suspension we need to receive communicate from you as soon as possible. As a reminder, your agreement with The Family Practice granted you the temporary rights to use The 100 Year Lifestyle® resources only in materials provided to you by The Family Practice, Inc. and only as long as your account is in good standing.

You must immediately stop using any and all of The 100 Year Lifestyle materials including the marketing campaigns, flyers, posters, fatheads, PowerPoint lectures and newsletters until your bill is paid. Use of The 100 Year Lifestyle logo, The 100 Year Lifestyle® or its trademarked phrases (i.e. Get Your ESS in Shape, Adjust Your Lifestyle) on your website, social media outlets or office forms for the purpose of promoting yourself or practice is no longer permissible. If you continue to use them you will be in violation of trademark laws with The Family Practice, Inc. and LCfE, LLC and will be held accountable. Also, you will no longer be considered a 100 Year Lifestyle Affiliate Office and your practice will be removed from the provider list on The 100 Year Lifestyle website. x You can continue to purchase The 100 Year Lifestyle books, The Longevity Solution and the Premium Sleep Pillow.

If you are a 100 Year Lifestyle Certified Provider or are in training to become a 100 Year Lifestyle Certified Provider, the provisions implied therein upon registration remain in place.
