CONTRACT BETWEEN JACKSON COUNTY SCHOOL BOARD AND CHEMICAL ADDICTIONS RECOVERY EFFORT, INC.

This contract is entered into between the Jackson County School Board and Chemical Addictions Recovery Effort, Inc. for the purpose of continuing ALPHA Program Services at Cottondale Elementary School.

The Parties Agree: I. Chemical Addictions Recovery Effort, Inc. agrees: A. To provide Prevention Services according to the conditions specified in Attachment I. B. Provide two (2) full-time counselors at the school site at no cost to the School Board. C. Provide a state-approved modular unit for housing the counseling services. D. Provide appropriate office and classroom furniture.

II. The Jackson County School Board agrees: A. To collaborate with CARE to provide the contracted services and conditions as described in Attachment I. B. Supply electric, water and sewage support for the modular unit. C. Supply all necessary boardwalks, stairs, and ramps to the modular unit.

III. The Jackson County School Board and Chemical Addictions Recovery Effort, Inc. mutually agree: A. All services shall be scheduled and approved by the Cottondale Elementary School Principal. B. All services will be conducted in accordance with Attachments I and II of this contract. C. Effective Date: 1. This contract shall begin on July 1, 2015. 2. This contract shall end on June 30, 2016. D. Representative of Contract: The representative for the Jackson County School Board for this contract is Carolyn Pilcher and/or Michael Kilts 850-482-1200 ext. 222 or 223. The representative for Chemical Addictions Recovery Effort, Inc. responsible for the administration of the program under this contract is Linda Wilson, Prevention Program Director, (850) 352-3553. E. Terms and Conditions: This contract and its Attachments I and II contain all of the terms and conditions agreed upon by the parties. The approximate value of these services and materials is $150,000.

RISK FACTORS FOR ADOLESCENTS APPROPRIATE FOR ALPHA

 Family history of alcoholism.  Family management problems.  Parental drug use and positive parental attitudes towards use.  Early antisocial behavior and hyperactivity.  Academic failure beginning in middle to late elementary school.  Little commitment to school.  Alienation, rebelliousness, lack of bonding to society.  Antisocial behavior in early childhood and/or adolescence.  Friends who use drugs.  Favorable attitudes toward use.  Lack of motivation and/or interest in school.  Truancy.  Ineffective coping skills.  Inability to interact appropriately with peers.  Family problems.

STUDENT NOT APPROPRIATE FOR ALPHA

 Special Learning Disabled (SLD).  Emotionally Handicapped (EH).  Severely Emotionally Disabled (SED).

CRITERIA FOR ADMISSION

1. The ALPHA Program is designed to serve elementary students identified to be “At-Risk” and who demonstrate the above indicators.

2. The student must be willing/able to attend at least two groups and one individual session per week.

3. Parental involvement is strongly encouraged. Attachment II

COOPERATIVE AGREEMENT

This Cooperative Agreement is entered into this 1st day of July, 2015 by and between the CHEMICAL ADDICTIONS RECOVERY EFFORT, INC. (herein CARE) and JACKSON COUNTY SCHOOL DISTRICT (herein agency).

CARE AND AGENCY agree to provide services for students referred to each other providing referred students qualify for services that CARE and AGENCY is/are authorized to provide.

CARE and AGENCY acknowledge that they are fully bound by the provisions of Federal Confidentiality Regulations which are explained in 42 CFR, Part 2, in receiving, storing, processing, or otherwise handling any information regarding students at-risk for chemical abuse.

CARE and AGENCY agree to institute and maintain appropriate procedures for safeguarding student information with particular reference to protecting identifying information; and, to ensure judicial efforts to obtain access to student information are consistent with specified guidelines outlined in 42 CFR, Part 2.

CARE and AGENCY agree to collaborate for students who need additional services.

CARE and AGENCY agree to cooperate with each other in their endeavors within the limits set by each organization.

CARE and AGENCY agree that this COOPERATIVE AGREEMENT will remain in effect until canceled by either party in writing.
