Job Description

Job Title A&E/Community Worker (Alcohol, Drug and Mental Health Community Diversion Service) Hours 18.5 hours weekly including participation in a weekend night shift rota. Status Start date 08.12.14. Fixed term to 31.03.15 Accountable to SWR Mind Chief Executive Responsible to SWR Mind Board of Trustees Main Base SWR Mind head Office, Eastfield, Scarborough (mental health worker) Salary £23,188 pro rata

Background Building on the success of The Cambridge Centre’s established A&E Alcohol Service and funded initially until 31.03.15 by Scarborough and Ryedale Clinical Commissioning Group (SRCCG) SWR Mind and The Cambridge Centre have come together to work to increase the capacity of Scarborough General Hospital A&E Department to support people with mental health and/or substance misuse problems within Scarborough Hospital A&E Department and in the community

Purpose of role To bring specialist drugs/new psychoactive substances (also known as ‘legal highs’) misuse or mental health expertise to the A&E/Community Alcohol, Drug and Mental Health Community Diversion Service.

1. To provide support to people with mental health and/or substance misuse problems presenting at Scarborough Hospital Accident and Emergency Department with the aim of:  Improving patient experience  Reducing attendances at A&E  Freeing up A&E staff who would otherwise be with these patients  Supporting improved discharge form A&E/Wards  To develop strong and positive relationships with the A&E team and provide training to A&E/Hospital staff as required  Delivering brief interventions within A&E when appropriate and consent given by patient

2. Where consent is given, to provide between 3 (substance misuse) and 12 (mental health) community follow up interventions with people who have presented at A&E or who are discharged from wards with the aim of:  Supporting clients to navigate health, social care and other relevant services in Scarborough and Ryedale  Supporting clients to find and use community services that will assist them in dealing with the issues in their lives that may be preventing recovery: o To improve mental health o To reduce substance misuse and associated risks o To engage individuals with their communities and long term recovery o To reduce attendances in A&E where alcohol, drugs, and or mental ill health is a contributing factor

 Improving patient experience  Freeing up A&E staff who would otherwise be with these patients  Supporting improved discharge from A&E/Wards  Providing training to A&E/Hospital staff as required

3. To provide this support as part of the Cambridge Centre A&E Alcohol misuse provision where people with mental health problems also have a substance misuse problems.

4. To develop age appropriate alcohol/drugs/mental health information packs that can be distributed to patients when they leave A&E

5. To develop referral pathways with partner agencies

6. To provide this support in line with employer policies and procedures.

7. This post is subject to an enhanced CRB check. 1. Policies and Procedures To ensure that all work is carried out according to employer policies and procedures, project rules and that all necessary paperwork is completed accurately and on time.

2. Service user assessment, support, review, and exit To support Project delivery through the assessment, support and review of service users within the project 2.1 Assessment – . Within A&E support medical staff to screen using the SAT tool to ascertain need. . To assess and evidence outcomes using TOPs, WEMWBS and the ‘Wheel’ tool as needed to assess the wants and needs of service users referred by the project, and to refresh this assessment as project process . Risk assessment – prior to undertaking work with a service user, ensure project risk assessment is undertaken and that all associated policies and procedures are followed

2.2 Support –  To develop supportive relationships with service users based on respect, trust and honesty  Through Projects Service User Handbook and/or guidance and information, to ensure that service users have explained to them all that is necessary to enable them to safely make the most of the project  Accompanied signposting – to ensure that, as far as is practically possible and where needed to achieve goals (as agreed in Support Plan), service users are accompanied/introduced to people, places and opportunities within their community. Accompanied signposting will also be used where the project is unable to provide direct support but another service may be able to.

2.3 Review – To review service effectiveness via TOPs, The Wheel and WEMWBS

2.4 Exit – To ensure that employer policies and procedures and project rules are followed when service users exit the service.

3 Support and Recovery Planning 3.1 Utilise all screening and assessment information to ensure support and intervention is provided quickly and efficiently in order to reduce immediate harm and appropriately engage with further services and the community as needed.

3.2 Structured Mental health Support: Following assessment, with the service user, discuss, develop, agree, implement and review a person-centred, recovery focused Support and Recovery plan. This includes updating of the Support and Recovery Plan where an individuals’ circumstances/need change unexpectedly.

4 Monitoring and evaluation To record data as requested for the purposes of monitoring and evaluation of the project

4.1 To keep accurate and up to date case notes

4.2 To ensure that such data collection is completed routinely and in line with Project paperwork, systems, processes etc.

4.3 As required, complete paperwork using agreed case management systems relating to each new/existing service user promptly and as required by the service and employer.

4.4 To provide reports for monitoring meetings

5 Administration – the post holder will: 5.1 Adhere to all employers’ policies and procedures and project rules relating to the effective and efficient administration of the project including but not limited to communication, data protection and management, health and safety, monitoring and evaluation

5.2 Ensure that all records are kept in the required manner

5.3 Be entirely responsible for managing their own on-line diary and project administration and keeping it up to date at all times

5.4 To be a key-holder 5.5 On request contribute information for SWR Mind quarterly newsletter, The Cambridge Centre Staff Briefing and regular blog

5.6 To attend regular formal and ad hoc supervision with your line manager

6 Developing and maintaining appropriate partnerships/networks, using these wherever possible and appropriate to effectively promote:

8 Communication

8.1 To ensure that all communication, both written and verbal, relating to this role is timely, clear and unambiguous.

8.2 To prepare for, attend and contribute to Employers Team meetings relating to the post. This includes, but is not limited to:  SWR Mind monthly Business Team meeting  SWR Mind weekly peer support meeting
