ACKNOWLEDGE RECEIPT OF THIS ADDENDUM BY INSERTING ITS NUMBER IN THE BID FORM; FAILURE TO DO SO MAY SUBJECT BONA FIDE BIDDER TO DISQUALIFICATION. THIS ADDENDUM FORMS A PART OF THE PROJECT DOCUMENTS; IT MODIFIES THEM AS FOLLOWS:

Addendum Item No. 1a

The underground storage system shall be Stormtech SC-740 or APPROVED EQUAL.

QUESTIONS AND ANSWERS

1. Question: Structure D11: Is this a Junction Box or a Drop Inlet? It is listed as both.

Answer: It is a drop inlet. Please see CG-7.0, D11 is listed as “PROP. D.I. (GDOT STD. 1019).

2. Question: Pipe conflicts. Some lines are RCP on the profiles but HDPE on the plan sheet. If the profiles govern, this presents another problem because not all of the profiles have been included.

Answer: The only discrepancies that I have found in the plan and profile sheets are in B1 to B2 and B5 to B6. For both pipe runs use 10” HDPE.

3. Question: 10”RCP: I have heard of 12”RCP but never 10”CP. 15”RCP is about the smallest diameter in this market. 12”RCP may be available; I just do not know of a producer.

Answer: Use 10” HDPE for B1 to B2 and B5 to B6.

FLAT CREEK STREAM RESTORATION – UPPER NORTH FORK FLAT CREEK Addendum 1-1 Issued on April 21, 2011 4. Question: The plan set I am using does not have drawings CD2.6 & CD 2.7. Mine has CD2.8 & CD2.9.

Answer: The e-mail Marilyn Hutchings sent out to all approved Bidders on Monday, May 23, 2011 should have corrected this problem. Originally Sheets 51 to 60 (CD1.7 to CD2.7) had been left out of the e-mailed documents that were sent to you at no cost. The printed sets that were sold for $75.00 had sheets 51 to 60 included.

5. Question: Can the DBE participation goals be clarified? The requirements in the EPA special provisions and the GA Recreational Trails Program section differ. Additionally, are any of the forms found within the EPA special provisions or the GA Recreational Trails Program section to be completed and submitted with the bid documents? It would be advantageous for us to have a checklist of required documents and forms for submittal at bid time.

Answer: Stream Restoration and Stormwater Treatment Bid Items fall under the EPD 319 Grant which are DBE 13.7% and WBE 7.0%.

Greenway Bid Items fall under the Georgia Recreation Trails Program which are DBE 10.0%.

Follow Section 00 21 13 Instructions to Bidders. There is a check list in this Section 00 21 13-4, a through g. DBE forms are not on the list, so don’t include the DBE forms in the sealed envelope. DBE information will be collected as part of the contract execution process.

6. Question: Will the remaining buildings within the project limits be removed by others prior to construction? There are notes in the plans that call for buildings to be removed if present at time of construction.

Answer: The buildings and concrete at 435 Banks Street will be removed by others. The removal of the metal building in the maintenance yard and the other concrete slab in this area need to be included in Bid item # 1 “Demolition”.

7. Question: Can the bid form be adjusted to allow the contractor to develop his own unit price for bid items 19, 20, 21, and 22? The unit prices included in the bid form are not accurate for the provided quantities. Also, is the excavated material referenced in pay items 21 and 22 required to be hauled offsite or may it be wasted within the limits of disturbance?

Answer: Change Item #21 to $35.00 CY. Leave 19, 20, and 22 as they are. Item # 21 and 22 can be wasted on site.

FLAT CREEK STREAM RESTORATION – UPPER NORTH FORK FLAT CREEK Addendum 1-2 Issued on April 21, 2011 8. Question: Will the contractor be responsible for any cost associated with re-locating power pole and guy wires for the project?

Answer: No, the City will pay for the power pole and guy wires to be relocated.

9. Question: On CG8.0 there is a note for fire hydrant and waterline valve to be relocated. Where is that located and where do we include it on the bid form?

Answer: The fire hydrant and valve are located at the corner of Pine Street and Industrial blvd. The City crews will relocate the fire hydrant and valve at no cost to the contractor.

10. Question: Can a mobilization line item be added to the bid form to cover incidental items that are required for the project? These items can be buried in the other line items, but if the City elects to delete certain line items from the project these incidental cost will be required or if the City adds certain items they would be paying more than need be.

Answer: No, any mobilization or incidental items should be included in the existing line items on the Bid Form.

11. Question: The demolition plan calls for the removal and replacement of the pavement at the road crossings of 1,660 SY. Please clarify if this quantity includes the storm crossings on the Roadways?

Answer: Yes, the pavement replacement for stormdrain crossing installation will be paid for through line item # 75 “Asphalt Paving”.

12. Question: Please clarify excavation/environmental disposal 400 CY. After reading the specifications this seems to cover a broad range of environmental issues. What is expected to be discovered as far as contaminated material? Is there a report other than the Asbestos Survey that was completed to determine what exist on-site?

Answer: The maintenance yard property is now in compliance with non-residential risk reduction standards. Three areas within the maintenance yard contained constituents above the approved risk reduction standards, and thus, were cleaned up. The constituents identified and cleaned up were as follows: Lead, Arsenic, Benzo(a)anthracene, bemzo(b)pyrene, and benzo(b)fluoranthene.

Any soil excavated in the maintenance yard will be placed in the fill areas along the existing loading dock wall that is to be demolished or in the berm areas where the underground storage is located. This soil will be encapsulated with 2 feet of clean

Bid Item # 17 “Excavation, Environmental/Special Disposal” is included in the Bid in case an unknown pocket of contaminated soils are encountered and need to be removed from the site and disposed of in an approved landfill.

13. Question: On CG 2.0 note 4 indicates that the contaminated material will not be hauled off but placed adjacent to an existing concrete retaining wall. Please clarify if this is correct, if so please identify where this location is.

Answer: See Answer to Question # 12.

14. Question: Please clarify what is included in the maintenance of the landscape for 1 year. Does this include complete mow, blow, and pine straw mulch periodically.

Answer: It includes maintaining plant life by watering or any other methods required to keep vegetation alive for one year. Any dead trees, plantings, live stakes, sod, etc. must be replaced. It does not include replacing pine straw or cutting sod during the one year warranty period.

15. Question: Is it the intention for all new plantings to have irrigation?

Answer: No, the irrigation will be installed in the maintenance yard area between Banks Street and High Street.

16. Question: It appears that CD 2.1-2.7 drawings were not received when plans were sent out.

Answer: See Answer to Question # 4.

17. Question: Do you have any specifications for Bid Items 93 – 104?

Answer: See Answer to Question # 4. Pages 51 to 60 has most of the details that you are looking for.

18. Question: Will the demolition contractor be responsible for removing and disposing the railroad ties at the time of building demolition?

Answer: Yes, include removing the railroad ties, mulch pile, logs, and any debris as part of Bid Item #1 Demolition.

Question: Bid items 89 and 90 – the item description simply states large or small deciduous tree respectively. Would it be possible to get a better definition of size requirement and species for these two line items?

Answer: In the Bid schedule, the large trees are 8’ and greater, including the evergreen trees. The small trees are 3’ to 4’ and 5’ to 6’.

20. Question: Could you please direct me to the detail for the bollard listed on line item # 93 in the Bid Form?

Answer: The detail is on page CD2.3 - 4

21. Question: Please specify a model number for the benches and trash receptacles from Victor Stanley.

Answer: Bench Model # FS-50 or approved equal. Trash Receptacle Model # RTC-40 or approved equal.

22. Question: In the pre-bid it was mentioned that the site excavation should balance. If that is the case do we need to include the 4,700 CY of import as it is a rather large quantity? Can the contractor adjust grades on-site to make the site balance in order to eliminate the import?

Answer: Part of this imported dirt will be used for the 2’ deep encapsulation in the maintenance yard area that has been discussed. The walking trail will need to be backfilled with imported dirt and I am sure that we will need some imported dirt in other places through out the project. The imported quantity in the Bid Form maybe more then is needed and if so the unneeded amount can be backed out at the unit price.
