IDAHO STATE UNIVERSITY PROFESSIONAL ACHIEVEMENT AWARD NOMINATION FORM

The Professional Achievement Award is presented to alumni of Idaho State University in recognition of significant professional achievement in a specific field or endeavor. Awards are given each spring at Idaho State University's Commencement. Nominations must be submitted no later than January 20th, 2018. All nomination packets are to be directed to the Dean of the college or division with which the alumnus being nominated is affiliated by degree.

Individuals selected for the Professional Achievement Award must be alumni who have exited or received their first degree from Idaho State University at least 10 years ago. This includes all individuals who have obtained at least 24 credits and who exited or graduated with a degree at least 10 years ago.

Qualifications for nomination:  Must be an alumnus of Idaho State University  Must show significant success and/or accomplishment in nominating field  Must have been out of school for at least 10 years

Criteria and materials reviewed by selection committees include:  A completed nomination form.  Three letters of support - one may be from the nominator. A current resume and/or vitae for the candidate and other supporting materials such as newspaper articles, publications, or other resources that speak to the candidate’s achievements.  Completed packets must be provided at the time of submission.

All documents pertaining to the nomination should be sent to the college of which the nominee is being nominated for. Colleges/units will notify Alumni Relations of their selection. Alumni Relations will compile all selected candidates and forward to ISU President’s Office for initial notification.

Some notes on nominations:  The burden for completing nomination materials lies with the nominators.  Nominators determine whether to inform a potential candidate of his or her consideration.  All nominations are to be forwarded to the respective college Dean or unit director for review and decision.  Any nominator whose candidate is not selected for the award in a given year must proactively re-nominate the candidate in subsequent years, if the nominator wishes the candidate to be considered again.  A completed packet including supplemental and updated information is required when re- nominating a candidate.

A selection committee in each college/unit will be formed at the direction of the Dean and each will review and vet all nominations received. The recommendations of the selection committee will be forwarded to each Dean, who will make the final determination of the award recipient. This process is strengthened when nominations are accompanied by support material. If possible, provide any or all of the following:  A completed nomination form (Required)  Three letters of support- one may be from the nominator  A current resume and/or vitae for the nominee  Newspaper articles, publications or other resources indicating candidate’s qualifications Support materials are equally weighed and should speak to a candidates:  Professional achievement or significant accomplishment in his or her field  Industry awards and special recognitions received in his or her field  Esteem held by colleagues in the field, as evidenced by letters of support
