Energy and Environment Cabinet Energy Leadership Award Nomination

For several years the Energy and Environment Cabinet has recognized energy leaders who have made a tremendous impact in Kentucky. Kentuckians have benefitted from these progressive leaders who have inspired others to save energy and look for alternative energy sources.

Below is a short application form to nominate a person, company or organization that has made great strides in conserving energy and/or finding alternative energy sources in Kentucky. Please be as descriptive as possible. Once completed, the form can be faxed to Susie Paul at 502-564-7484, e-mailed to [email protected] or mailed to Department for Energy Development and Independence, 500 Mero Street, 12th Floor, Frankfort, Ky 40601.

Please make sure the department receives your nomination by August 21, 2015.
