Facilities Use Request

With many users at each of our farm/dairy/field facilities, it is critical to ensure that we have knowledge of all ongoing and desired activities in order to ensure compatibility, and to coordinate resources required to provide support and facilitation as needed. Importantly, we have responsibility for compliance with federal, state, university and local regulations, and to ensure safety for our staff, students, faculty, and visitors.

Please complete the following information well in advance so that we can evaluate our abilities to accommodate your requested use of facilities and resources. The completed form in digital format (not paper) should be emailed to the relevant manager(s) and the Faculty Fellow; contacts are listed below. Note that a separate application should be used to request pace in the Macfarlane Research Greenhouses. http://www.colsa.unh.edu/aes/forms2

You may be asked to visit with the Facility Manager(s) and/or the NHAES Director’s Office to clarify intended activities and requirements so that we may better serve your needs. The Facility Manager(s) and Director’s Office will jointly evaluate resource availability including potential impacts on existing and planned activities, budgetary ramifications, and related management considerations. Among these are any potential implications on organic certification (i.e., several farm facilities, forage fields, etc.). This may require interaction with State of NH organic certification personnel. The NHAES Directors office will notify the requestor of a decision along with any qualifications, stipulations and/or requirements. Once evaluated, a signed copy will be returned to you, and another kept for our records.

Please be aware that full adherence to all relevant regulations, institutional approvals and compliance requirements (for example, http://www.unh.edu/research/compliance-safety) is the responsibility of the individual facility user. Supporting documentation must be provided before any use is permitted.

Requestor information:

1. Describe the desired usage of the facility or facilities, including inclusive dates/timeframe for the activity. What is the desired amount of land, number and types of animals, greenhouse bench space, or other needs for the activity?

2. Is this part of a supported activity such as research, teaching or Extension? What is the supporting entity? For extramural or NHAES research grant provide specifics about the funded project, for teaching provide class name, number of students, and similarly for Extension activity, etc. 3. What are required treatments, preparation, applications or modifications to land, greenhouse space or animals? Provide a summary of your intended measurements or sampling. What types of staff support are required?
