Allen Park Public Schools Athletic Participation Fee Application 2014/15 Fee (Per Sport) – High School/Community School $110 ($275 max per athlete) Middle School $60 ($225 max per athlete), District Wide $330 per family We invite you to take advantage of a unique learning experience by participating on an athletic team. This learning experience is unique because it offers educational opportunities that are not readily available in the classroom. By participating, our student-athletes learn the necessary skills to work effectively with others, solve problems, and be productive and contributing members of society. These skills are consistent with the Mission of the district, and the Athletic Mission Statement found in the Athletic Code of Conduct.

I have reviewed the “Athletic Fee Program” and understand that the fee I am paying ($110 for High School/Community School Applicants and $60 for Middle School Applicants) does not guarantee playing time, control over any conditions of the team, and is not refundable except as indicated. In addition, I understand that the fee will not be refunded or pro-rated for an athlete who becomes ineligible during the season, when an athlete is removed from a team for disciplinary reasons, when cancelled contests cannot be rescheduled, or when a full allotment of games cannot be scheduled. In addition, I understand that the number of scheduled contests may vary from level to level and from school to school. I also understand that paying the fee does not alter the Student Policies of the Allen Park Board of Education, the Athletic Code of Conduct, the regulations of the Michigan High School Athletic Association, and individual team rules.

ATHLETIC FEE – FINANCIAL AID APPLICATION

THE ATHLETIC PARTICIPATION FEE IS DUE NO LATER THAN THE CONCLUSION OF THE FIRST WEEK OF PRACTICE FOR THE PARTICULAR ATHLETIC PROGRAM

Students who would like to participate in athletics but are unable because of financial problems have the option of applying for financial aid. The guideline used for granting financial aid is the Free and Reduced Lunch Program. Students are eligible for financial assistance only if their family qualifies for this program, and only if the information from the program is allowed to be used by the Athletic Department. Students who are on the Reduced Lunch Program will receive a one-half reduction in their Athletic Fee. Students who are on the Free Lunch Program will have their fee waived. The established fees for the school year are: $110 for high school/community school participants, and $60 for middle school participants. Those who wish to apply should complete this form for each sport in which they would like to participate.
