Give an Exceptional Student or Star Educator much-deserved arts recognition! Submit a nomination for a 2011 Rockford Area Arts Council’s, Mayor’s Arts Award. Nominations are due to the Arts Council no later than February 11, 2011

Awards will be presented at the Family Arts Festival, Riverfront Museum Park, Kresge Hall, 711 N. Main St, Rockford, IL Sunday, March 13, 2011 @ 2 p.m.

Nomination Information For Rockford Area Arts Council’s Mayor’s Arts Awards Exceptional Student and Star Educator

Nominees will be evaluated on the basis of the breadth and depth of their work. A panel will review nominations based on the four evaluation criteria below.

Award Categories

1. Exceptional Student of the Arts Award: Honors a high school-aged student’s artistic talent and community involvement in the arts during the student’s high school years.

2. Star Educator, Specialist, Instructor: Honors an educator, specialist or instructor who has enriched lives, inspired excellence, introduced children to the arts or expanded opportunities for children to learn through the arts.

How To Submit A Nomination Anyone may submit a nomination and may cast nominations in both categories. Preference will be given to those who have not received an award in the past. You may copy this nomination form. Nominations must be received by 5 p.m. February 11, 2011. Awards will be presented at the Family Arts Festival, Riverfront Museum Park, Kresge Hall, 711 N. Main St, Rockford, IL on Sunday, March 13, 2011 @ 2 p.m.

A complete nomination packet consists of: • A completed nomination form. • Letter from nominator. • Two letters of support describing how your nominee exemplifies the criteria. • Digital photos, digital video footage of your nominee’s work (if applicable).

Evaluation Criteria Four criteria will be used for evaluation, though not all criteria apply to both award categories.

1. Describe the artistic medium of the nominee. Describe the quality of the nominee’s work in this discipline. 2. What are the student’s or educators accomplishments as they relate to the arts? 3.
