 Call Meeting To Order – 1:38 pm

 Once Around the Table  Went once around the table and everyone introduced themselves.

 Approval of Notes  Nancy made a motion to approve the November minutes with the correction of one minor typo. Adding a space between the “r” in “October” and the “1” in the date of the “Approval of Notes” section from the November minutes.

 Review Agenda  No updates to the agenda were necessary. Nancy made a motion to approve December’s agenda.

 Promotions of Safety Incentive Programs  The email reminders we are sending out seem to increase participation in the incentive programs. An email will go out to all the staff to recognize the winners of December’s Safety Incentive Program drawings.

 Reports  Budget Report – Ashley Lightfoot  Ashley provided a full report and spreadsheet with explanations of entries.  Current Account Balance  $7,325.50  Received Income  $31,000 from all sources.  Still waiting on $24,581.25 from the SIPE Rebate and SIPE Grant.  Budget Expenditures – Ashley Lightfoot  Encumbrances/Expenditures total $23,674.50 thus far.  SIPE Credit  Still waiting for credit from SIPE.  SIPE Grant Funding  Still waiting for SIPE approval on the $11,400 forklift purchased by the Maintenance Department.  Site Inspections  All departments have completed their inspections and have been approved to spend their allotted $400. If the money is not spent, it goes back into the Safety Committee.  Discretionary Funds  Ashley stated that an oxygen cylinder cart and a stand-up desk were purchased.  Review Employee Accident Trends – Karen Woodruff  Total of 18 since July 1, 2015  Reported Injuries – 11  Claims – 7  Only 3 additional reports, no additional claims since last month.  Student Accidents – Ashley Lightfoot  Three student incidences were reported. Two injuries due to cuts at Mesa View, and one seizure at Loma Vista. There was one incident of a broken hand today at Loma Vista that is not on the report.  Safety concerns/suggestions:  Holly stated that she is very concerned about staff safety in the Medically Fragile classrooms in regards to their lifting procedures. They had a lifting class recently, presented by SIPE, however, there are discrepancies in the staff job descriptions regarding lifting requirements. Karen stated there is a lifting safety video that is waiting to go to Cabinet for approval. Holly expressed a concern with the staff turnover and ensuring that the new employees are up-to-date on the appropriate safety techniques. Karen informed her that it is up to the managers to contact Ashley when new employees are hired.

 Safety Inspections, all sites and departments – Ashley Lightfoot  Every department has completed their Site Safety Inspections and have earned their $400 incentives.

 Risk Management / SIPE – Garth Maijala  No SIPE representative was present.

 Old Business  Safety Committee Meeting Locations  Everyone agreed to keep the meetings where they are currently scheduled.  Add a Safety Portion to SLOCOE Newsletter  Will be discussed with HR/Admin (Susan, Angie, etc)  Annual Blood Drive – January 27, 2016  Karen reminded everyone that the annual blood drive will be held on January 27, 2016 from 12:00-4:00pm. She has already sent posters out and she will send out a reminder next week and also in January. Jamba Juice cards will be given to participants.

 New Business  Purchase Requests:  Ashley presented one new Purchase Request to purchase a head sling for students in the Medically Fragile Classroom at the cost of $241.60. The sling would provide head support for the student, which would free up the staff to help in other ways. Staff currently needs to support the student, the student’s head, and the oxygen tank and the tubing. Ashley felt SIPE would not agree to the request and felt like it would fall more firmly in the category of the Safety Committee Coordinator Discretionary Fund. Karen made a motion to approve the request. Steve seconded that motion.  Department Safety Meetings:  Karen stated they discussed material handling and slips, trips, and falls, and they signed their sign-in sheets. Nancy stated they received the link and shared the info with their staff. Traci stated her department does the same thing. Robin stated she is not aware of how Alt-Ed does their safety meetings as she is not present when/if they occur.

Statement of Purpose and Objectives: The Health Management and Safety Committee will assist supervisors, site administrators, and employees in creating a climate of safety awareness. This requires open lines of communication where employees have the opportunity and are eager to participate in safety programs. Employees are encouraged to voice concerns about hazards or potential hazards without fear of reprisal.

Committee Goal for 2015-16: Our goal for 2015-16 is to facilitate a 50% reduction in “slips, trips and falls” at SLOCOE, increase awareness of safe practices, and to see an overall increase in employee participation in safety incentives.
