As part of the sleep medicine certification procedures in the Nordic countries the Nordic association for Sleep Research and Sleep Medicine (Iceland, Norway, Sweden, Finland, Denmark) has decided to initiate the process by inviting active and experienced sleep practitioners to apply for certification as sleep specialist during a transitional period.

You have been recognized as a candidate for this by the Board of the Swedish Sleep Research and Sleep Medicine Society and are invited to apply for certification.

Your application will be granted and certification issued if your theoretical and practical knowledge comply with the requirements listed in the attached questionnaire.

If there are issues you do not have sufficient knowledge of the National Sleep Research and Sleep Medicine Society will assist you in acquisition of this knowledge during a transitional period of no longer than 18 months.

Appendix 1. Index of preceding practical experience

The candidate will be required to provide a written declaration from the head of the SMC where training was completed stating correct and independent performance of the following:

 full polysomnography (PSG) recording procedure in 30 patients;  reading of 100 PSGs, comprising a spectrum of neurological, cardio-respiratory and psychiatric disorders in both adults and children;  full MSLT/MWT1 procedure in 10 patients;  if applicable to current practice of sleep medicine in a particular country, 50 ambulatory cardio-respiratory recordings; and stating  treatment experience in 100 patients with sleep/wake disturbances, including cases of each of the following: sleep related respiratory disturbances (including nasal ventilation therapy), other intrinsic sleep disorders, and sleep disturbances due to physical and mental disorders.

The candidate applying for the sleep medicine qualification must demonstrate knowledge of the following sleep medicine topics:

2.1 EEG and physiological basis of sleep:

 physiological and age-related variations of sleep/vigilance during the day, examination and evaluation of vigilance disturbances, physiological variations, age related variations;  physiological and biochemical changes in sleep;  models of sleep ontology and function;  EEG activity during sleep, staging of sleep;  effectiveness and limitations of the conventional sleep staging classification;  particular aspects of REM and NREM sleep: - EEG activity; - motor-neural activity; - sensory activity; - activity of the autonomic sympathetic/ parasympathetic nervous system; - heart and circulatory functions; - respiratory functions; - metabolic activity; - thermoregulation;  regulation of hormones during sleep and their dependence on sleep stages;  basic knowledge of mental activity during sleep including dreams;

2.2 Chronobiological aspects of sleep:

 circadian rhythm and its influence on temperature, cardiocirculatory function and hormone secretion;  chronobiological models of sleep regulation;  circadian variation of cognitive performance and concentration;  tiredness and sleepiness during the day;  methods for recording time dependent variations of sleepiness and alertness;  diagnostic procedure for assessment of circadian variation and physiological variables;  sleep disturbances due to disrupted sleep-wake rhythm, their classification and differential diagnoses;  management of circadian rhythm disorders;

2.3 Diagnostic procedures for identification and assessment of sleep disturbances:

 outpatient diagnostic procedures for insomnia;  sleep related respiratory disturbances;  sleep related circulatory disturbances;  outpatient methods for identifying and assessing disturbances of vigilance;  knowledge of validated questionnaires for assessing sleep disturbances;  diagnostic procedures including: - PSG; - cardio-respiratory polygraphy; - MSLT and MWT; - rating scales and neuropsychological tests;  staff, technical and logistical prerequisites for running a sleep-lab (cf. European Guidelines for the Accreditation of Sleep Medicine Centres);

2.4 Positive and differential diagnosis of sleep disorders as listed in ICSD-2

 differential diagnosis and classification of sleep disturbances, daytime sleepiness and tiredness; in particular, the candidate must be able to recognise the symptoms, differential diagnosis and prognosis of the following disorders: - insomnia; - sleep apnea syndromes and other sleep-related breathing disorders; - hypersomnia; - narcolepsy; - parasomnias; - sleep disturbances in various medical and psychiatric diseases;

2.5 Therapy of sleep disturbances:

 sleep hygiene;  influence of medication on normal sleep;  drug therapy for sleep disorders;  cognitive behavioural therapy and other psychotherapeutic procedures for sleep disturbances;  nasal CPAP and other aspects of non-invasive ventilation;  surgery;  dental appliances.
