5. Condition: [ ] Stable [ ] Fair [ ] Serious [ ] Critical 6. VS: qid with blood pressure sitting and standing. Weight on admission and each AM. 7. Activity: Up with assistance. 8. Nursing: I/O Q shift. 9. Diet: NPO after midnight for a possible EGD tomorrow. Untilthen she may have clear liquids as tolerated. 10. IV: IV LR at 120 ml/hour , with 20 mEq of KCl added per liter of IV fluids. 11. Meds: Tylenol 1000 mg PO Q 4 hr prn pain. Imodium, 2 capsules by mouth after the stool specimen has been obtained, and then one capsule by mouth after each loose stool, up to a maximum of 8 capsules per day. Ambien 5 mg, one tablet by mouth at bedtime and may repeat X 1 if needed for sleep. For nausea as needed use the following drugs: Reglan 5 to 10 mg IV every 6 hours. Zofran 4 mg IV every 6 hours. The Reglan and the Zofran can be alternated every 3 hours to relieve nausea as needed. Protonix 80 mg IV, and then give 40 mg IV every 12 hours. Carafate 1 gram/10 ml, 2 tsp by mouth 4 times a day (or before meals and at bedtime). 12. Other Meds: 13. Labs: CBC, chem 7, LFTs, amylase, and lipase; Stool studies for occult blood, WBC stain, C-diff toxin screen, stool culture, rotavirus screen; and OVAS (screening test for ova and parasites in the stool). Repeat CBC and chem. 7 daily. 14. Other: Call MD if: altered mental status, T 102F or higher, chest pain, pulse < 40 or >130. 15. H&P: Type up the dictated H&P. 16. Consultants: Dr. Davis for an EGD.
