A. Personal Statement I have been working as a psychiatrist in a Federally Qualified Health Center (FQHC) locate in a rural underserved community by the US-Mexico border for more than 10 years. Due my clinical experience and the observed health disparities, I went back to school and obtain a Master's in Public Health focusing in Community Health Sciences as well as a T-32 in Behavioral Medicine and Epidemiology sponsored by NHLBI. I have worked as a sub-investigator in a R-34 (PI Chavira) that examined lack of anxiety treatment among Hispanics living in this underserved area. I have published in the area of Hispanic health disparities as well as epidemiological aspects of mental health with comorbid medical conditions, frequently encountered in our FQHC. My goal is to continue building an expertise in mental health disparities among individuals living in rural underserved communities. Due to my work as clinician and investigator in an underserved area, I find crucial to build partnerships and research projects with scientists that have a build grand portfolio in this topic. As a Co-PI I will continue contributing to the scientific literature about the importance of improving mental health treatment and interventions in rural communities by the US- Mexico border.

C. Contribution to Science (includes relevant publications for this grant) Mood disorders, temperament and comorbidities. This clinical research interested stemmed from the complex patients treated in community health centers. These cases are the norm not the exception. Novel treatment options are constantly needed. Some treatment options such as pharmacological combinations are not possible to be tested in clinical trials due to cost and feasibility. Association of medical conditions with mood and anxiety disorders. My clinical research interest has focused on the different biological aspects that are associated with mood and anxiety disorders. I have studied the potential association of platelets with psychiatric disorders. Also I have looked at the association of inflammation biomarkers with symptoms of depression in a large epidemiological cohort. 4. Camacho A, Dimsdale JE. Psychiatric Disorders among Hispanics living in Rural Underserved Communities Over the last 10 years parallel to my clinical practice in an underserved community by the US- Mexico border, I have developed an interest in clinical research. With my colleagues and collaborators we have demonstrated that Hispanic/Latinos living in this underserved community rely on family support and that stigma associated with mental illness hinders access to adequate care and prevention. Furthermore, we studied the use of mental health professionals (community health workers) to treat depression among patients attending a community health center in urban Southern California. These community health workers were licensed health professionals from Mexico. They received training in evidence based problem solving therapy. We demonstrated that due to cultural beliefs, Hispanic/Latinos have difficulty abiding to traditional forms of delivering therapy. This work as helped to expand our clinical research interest and look for alternative health providers that could deliver adequate psychosocial interventions. We learned that Latinos living in underserved community relay on folk medicine and have their own cultural beliefs about mental

2 health symptoms. Our aim is to continue studying different culturally-tailored interventions that are sustainable and could improve mental health symptoms among patients living in rural underserved communities. 7.
