TITLE: DIABETES EDUCATION

PURPOSE: To establish a system and resources to provide basic education to newly diagnosed diabetics.

POLICY: Basic education for newly diagnosed diabetics will be provided by nursing staff, utilizing the following procedure and tools:

PROCEDURE:

I. PHYSICIAN’S ORDER

A. Obtain a physician’s order for diabetic education for all new diabetics for self care of their diabetes. This order should be obtained and initial education initiated as soon as you know that a patient is likely to require checking their blood glucose levels upon discharge.

B. Obtain a prescription for a glucometer (specify “Freestyle”), if the patient will require blood glucose testing at home.

1. Have the family obtain the glucometer from an outside pharmacy and bring it to the hospital.

2. If it is not possible for the family to obtain the glucometer, contact Nursing Supervisor, Dietician, or Case Manager.

II. INSTRUCTIONAL MATERIALS

A. Obtain instructional materials and samples from a bin labeled Diabetes Education Information, located on the Diabetes Education Shelf in the Med Room in Station 2 and give to the patient.

B. Allow the patient to view video material (“Basic Skills” for English speakers, “Game Plan” for Spanish speakers). NOTE: These DO NOT go home with the patient. Return them to the Diabetes Education Shelf.

C. Review all material in the Diabetic Teaching Checklist (Attachment A), initialing each item on the form as you MENDOCINO COAST DISTRICT HOSPITAL GENERAL PATIENT CARE POLICY AND PROCEDURE STATEMENT

progress. Utilize the Diabetic Teaching Checklist Resource Information (Attachment B). Report to the oncoming nurse at change of shift report what material you have covered so that the training can continue on all shifts.

D. Return demonstration by the patient is required for Insulin Injection and How to use a Monitor.

E. The Diabetic Teaching Checklist Resource Information is to be sent home with the patient.

III. DOCUMENTATION

A. Document the patient education provided by completion of the Diabetic Teaching Checklist. This form is a part of the permanent medical record (Education Section).

B. Document return demonstration by the patient on the Diabetic Teaching Checklist for Insulin Injection and How to use a Monitor.

C. Document referral for additional follow-up as ordered by the physician. Attachments: A. Diabetic Teaching Checklist B. Diabetic Teaching Checklist Resource Information
