TRAVEL REQUEST FORM INSTRUCTIONS

INSTALL ADOBE ACROBAT XI PRO Ensure that you have Adobe Acrobat XI Pro installed:  Select start button > click in search bar > type in Adobe If Adobe Acrobat XI Pro is not installed:  Go to the STAFF page on the FBISD website  Click on Financials/CRM > Sign In > CRM > Customer Support Center > Create a Case  Create a case requesting the Adobe software update

ACCESS TRAVEL FORM  Open Internet Explorer (do not use any other browser)  Go to the Fort Bend ISD website  Click on Departments & Services > Career & Technical Education > Resources > Teacher Resources > Travel Request Documents (always use most recent file on website) o http://www.fortbendisd.com/cms/lib09/TX01917858/Centricity/Domain/72/TravelRequestForm2015.pdf  SAVE Travel Form before you enter any information

COMPLETE TRAVEL FORM  Complete all fields, use N/A if leaving a field blank  By hovering over each field, further explanation about the field will appear  As the teacher, your signature is required on the left hand side of the voucher under “Authorization for Travel”.  Create a signature by following the steps Adobe provides. After the 1st time of creating a signature, you will only need to remember your password.  SAVE the file

HOW TO SEND THE TRAVEL FORM FOR SIGNATURE VIA EMAIL  Open your email and address the subject line to [email protected]  Subject Name should be “LAST NAME_PROGRAM AREA_TRAVEL DATE” (ie. INDELICATO_AG_121015)  Attach Travel Request Form, additional Vouchers, Agenda, Registration, Housing Form, Government ID if they are applicable either as separate files or as ONE PDF  SEND to [email protected]

How to Create Single PDF File

o SAVE Travel Request Form o Click on FILE in Acrobat o Click CREATE o Click COMBINE FILES INTO A SINGLE PDF o Click ADD OPEN FILE o Click ADD FILE (for additional documents such as voucher, agenda, etc. o Click COMBINE FILES o SAVE file

RECOMMENDATIONS  As we try to automate the travel process, it is highly recommended that you walk your administrator through signing the document for the first time.  It is also helpful if you explain to your administration that this document replaces all district and campus forms per the Business & Finance department. Those forms include:  Remember that once you print the document, you will not be able to provide an electronic signature and a handwritten signature is then required.
