A. Did unexpected adverse events occur resulting in increased pain, distress or death rates to live vertebrate animals in the study? If so, how were they managed and what steps were taken to prevent recurrence (if applicable)?

B. Do you have changes in 1) personnel or 2) modifications to this protocol that you wish to submit with the annual continuation? ☐ YES ☐ NO If yes, fill out the appropriate sections in the following pages. In no, complete this page alone.

1) UTC-IACUC Personnel Amendment

Please make the following personnel change(s) to this protocol: CITI completion date or Procedures this person will perform Add Delete Name, Degree date of most recent and their experience mandatory training 1

* Individual training records should be maintained with the individual’s name, the procedures you have trained this individual to perform, and the date. List all specific procedures from the protocol that new personnel will perform. (Including euthanasia, administration of anesthesia, specific surgical procedures, monitoring of animals after surgery, administration of substances, etc.) Be advised that the P.I. must assure that all persons participating have demonstrated competence required to perform these techniques. Each individual is required to complete the on-line CITI training courses and pass the online examination. If additional rows are needed, highlight the bottom row of the table above and paste.

2 2) UTC-IACUC Protocol Modifications 1. Change(s) to be made: (If you are requesting additional animals, please provide responses to questions 3 and 4.)

2. Reasons for change(s): (Please be as thorough as possible while providing information, if necessary, regarding how this amendment alters the overall goal of the protocol.)

The addition of new procedures that may cause pain or distress requires a new alternative search. The Animal Welfare Act regulations require principal investigators to consider alternatives to procedures that may cause more than momentary or slight pain or distress in a human being to which that procedure is applied. The UTC-IACUC believes that the performance of a database search remains the most effective and efficient method for demonstrating compliance with the requirements to consider alternatives. Database 1 Name of database searched: Click or tap here to enter text. Date of search (must be within 6 Click or tap here to enter text. months of protocol submission): Years covered by search: Click or tap here to enter text. Search strategy (must show how Click or tap here to enter text. keywords were combined): Other sources consulted: Click or tap here to enter text.

When searching for alternatives the following should be addressed: • Replacement of existing animal methods with non-animal methods whenever possible. • Reduction of the number of animals needed. • Refinement of research procedures to minimize pain and discomfort.

For each painful procedure, were alternatives identified? ☐ Yes ☐ No

If “Yes”, please describe:

If “No”, skip to Question 3.

If alternatives were identified, will they be incorporated into your proposed work? ☐ Yes ☐ No

If “No”, please explain?

3.Will there be an increase in animal numbers? ☐ Yes ☐ No

3 If “Yes”, complete the table below.

Number in the original Source of the additional Species to be added Number to be added protocol animals

4. Justification for additional animals: (Please be as thorough as possible.)
